Hemodiyaliz uygulanan hastalarda eritropoietin kullanımının depresyon, anksiyete ve yaşam kalitesi üzerine etkileri

Kronik böbrek yetmezliğinde başta gelen tedavilerden biri hemodiyalizdir. Bu alanda yapılan ilerlemeler ile hastaların yaşam süreleri uzamış ve artık psişik ve sosyal sorunlar konuşulur hale gelmiştir.
Anemi hemodiyaliz hastalarında önemli bir sorun olup, psişik sorunlar yaratabilmekte ve yaşam kalitesi üzerinde olumsuz etkiler oluşturabilmektedir. Eritropoietin böbrek yetmezliğinde oluşan aneminin tedavisinde yaygın olarak kullanılan bir ilaçtır.
Eritropoietin tedavisinin hastanın yaşam kalitesi, egzersiz kapasitesi, iştahını ve bilişsel fonksiyonlarını etkilediği gösterilmiştir.
Çalışmaya bu amaçla 31'i eritropoietin alan 27'si eritropoietin almayan hemodiyaliz hastası dahil edildi. Hastaların depresyon ve anksiyetelerini ölçmek amacıyla Durumluk ve Sürekli Kaygı Ölçeği, Beck Depresyon Ölçeği ve yaşam kalitelerini ölçmek amacıyla Sheehan yaşam kalitesi ölçeği uygulandı.
Yapılan istatistikler ile gruplar arasında depresyon ve anksiyete ölçekleri arasında anlamlı fark tespit edilmekle birlikte yaşam kalitesi ölçeğiyle anlamlı fark tespit edilmedi.
Elde edilen sonuçlar hemodiyaliz uygulanan hastalarda eritropoietin tedavisinin mental fonksiyonlar üzerinde olumlu etkileri olduğunu göstermiştir.
The effects of erythropoietine on the depression, anxiety and quality of life under hemodialysis
Hemodialysis is one of the fundamental therapies of chronic renal failure. After the development on this method and the increase of the survival of patients, psychological and social problems have become matters of research in the recent [year]s.
Anemia, one of the important problems of dialysis patients, affects the patient? psychiatric state and quality of life. Erythropoietine is a useful drug for the treatment of anemia, widely used in these cases.
It has been shown that erythropoietine therapy can affect the quality of life, exercise capacity, appetite and cognitive functions.
In the present study, 31 erythropoietine-receiving patient and 27-dialysis patient not receiving the drug were included. The State and The Trait Anxiety Scale, Beck depression scale and Sheehan life quality scale tests were carried out in order to measure the patient' degree of depression and anxiety.
There has been a significant difference on the depression and anxiety scales between the groups; whereas no difference has been observed in Schehean life quality scale.
In conclusion, the therapy with erythropoietine could improve the mental functions of patients under hemodialysis.
