Diensefalik ( talamik-subtalamik) lezyonlu hastalarda, lezyon bilateral olsa bile, koma sıklıkla geçicidir; çoğunlukla takiben somnolans görülür. Diensefalik ve mezensefalik pontin lezyonlu hastalarda şuur bozuklukları tegmentumdaki asendan retiküler aktive edici sistemin direkt hasarı ile ortaya çıkar. Mezensefalik pontin lezyonlu hastalarda, kalıcı stupor veya koma hipersomniye göre daha sıktır ve genellikle bilateral tegmental hasarı telkin eder. Derin stupor'a nöro-oftalmolojik bulguların eşlik ettiği; lateralizasyon bulgusu olmayan hastanın kranial BT'sinde her iki talamus paramedian bölümünden başlayarak, 3. ventrikül yan duvarları boyunca mezensefalon tegmentumuna uzanan infarkt alanları saptanmış olup, bir yılı aşan gözlem boyunca klinik tablo değişmeden kalmıştır.

In patients with diencephalic (thalamic-subthalamic) lesions, even if bilateral, coma often is only transient, evolving frequently to a sleeplike somnolence. In patients with diencephalic and mesencephalic pontine lesions, disturbance of conciousness is due to direct disruption of the ascending reticular activating system in the tegmentum. In patients with mesencephalic pontine lesions, persistent stupor or coma is more frequent than hypersomnia, usually implying bilateral medial tegment damage. In the case with neuro-ophtalmologic signs accompanying deep stupor,

infarct zones starting from the bilateral thalamic paramedian parts through the 3rd ventricle lateral walls down to bilateral paramedian mesencephalic tegmentum have been observed in the cranial CT of the patient who had no laterality sign. Neurologic findings of the patient throughout the observation over a [year] have remained constant.
