Panik bozukluk, sekonder depresyon ve deksametazon supresyon testi

Depresif bozukluklarla,anksiyete bozuklukları arasındaki ilişki uzun bir süredir bilinmektedir. Ancak,bu ilişkinin doğası tamamıyla açıklanamamıştır.Çalışmalar panik bozuklukla(PB) depresyon arasında yüksek komorbiditeyi göstermektedir. Bu komorbidite bu iki rahatsızlık arasındaki biyolojik ilişkiyi düşündürmektedir.
Deksametazon Supresyon Testi (DST) depresif hastaların %50 kadarında pozitiftir.Diğer taraftan,panik bozuklukta DST sonuçları ile ilgili gelinmiş ortak bir nokta yoktur.Bir grup çalışmada,PB'daki DST sonuçlarının, Major Depresyon? dakilerle (MD) benzer olduğu bulunmuştur. Çalışmamızda PB'ta Limbik-Hipotalamik-Hipofiz-Adrenal (LHHA) aksını ve klinik bağlantılarını bulmayı hedefledik.
Çalışmamızda,MD'lu grupta DST nonsupresyonunun PB grubundan daha yüksek olduğunu bulduk. DST sonuçları her iki grupta farklı olmasına rağmen bu bozukluklar LHHA aksı açısından ortak bazı noktalara sahiptirler.
Sekonder depresyonlu PB ilesekonder depresyonsuz PB arasında istatistiksel bir korelasyon yoktu.Ancak Her iki grupta DST sonrası kortizol seviyeleri, Beck anksiyete ve depresyon skorları açısından istatiksel olarak farklılık vardı.

The relationship between depressive disorders and anxiety disorders has been well known for quite long time.However,the nature of the relationship is not perfectly explained.The studies reveal high comorbidity rate between depression and panic disorder(PD).Such a comorbidity raise the question of biological interrelationship of these disorder.
Dexamethasone Suppression Test (DST) is positive up to 50% of the depressive cases.On the other hand, there is no consensus regarding DST results at panic disorder.Some studies result in similar DST results in PD as in major depresyon(MD).However, DST results did not differ from normal values at the other studies. Our study aims at exploring Limbic-Hypothalamic-Hypophisis-Adrenal(LHHA) axis in PD and its clinical correlates.
We found higher DST nonsuppression at MD cohorts than PD group. Although, DST results are different in both groups, the disorders have common points as regard to LHHA axis.
There was no statistical correlation between PD with secondary depression and PD without sekondary depression.However two groups were statistically different in terms of post DST cortisol levels,Beck anxiety and Beck depression scores.
