Gebelik sırasında şiddetli psikiyatrik rahatsızlıkların farmakolojik tedavisi, tedavinin fetüse potansiyel zararlı etkileri nedeniyle karmaşıktır. Hem annenin hastalığının hem de verilen ilaçların fetüs üzerinde olumsuz etkileri olabilir. Gebelik döneminde verilen psikotrop ilaçlar, kullanıldığı gebelik ayına göre fetüste değişik yan etkilere neden olabilmektedir. Özellikle ilk 3 ayda alınan psikotropların fetüste teratojen etkilere daha fazla neden olduğu bildirilmektedir. Bu nedenle gebe kadınlarda elektrokonvülzif terapi (EKT) önemli bir tedavi seçeneği haline gelmekle birlikte pek çok psikiyatrik bozukluğun tedavisinde de etkili ve güvenli bir seçenektir.
