Bir üniversite hastanesi psikiyatri polikliniğine başvuran konversiyon bozukluklu hastaların sosyodemografik ve klinik özellikleri

Amaç: Bir üniversite hastanesi psikiyatri polikliniğine başvurmuş ve klinik görüşme sonucu konversiyon bozukluğu tanısı
konmuş hastaların sosyodemografik ve klinik özelliklerinin araştırılması amaçlanmıştır. Yöntem: Bu çalışmada Celal Bayar
Üniversitesi Hastanesi Psikiyatri Polikliniği’ne Şubat 1995-Eylül 2000 tarihleri arasında başvuran ve yapılan klinik görüşme
sonucunda DSM-IV tanı ölçütlerine göre konversiyon bozukluğu tanısı konmuş olan 227 hastanın poliklinik kartları incelenmiştir.
Sonuçlar: Çalışmaya alınan hastaların %88.1’inin kadın, %39.2’sinin 15-24 yaş grubunda, %52.4’ünün ilkokul mezunu,
%61.2’sinin ev hanımı, %64.8’inin evli olduğu ve %59’unun kentte oturduğu belirlendi. Erkeklerin yaş ortalaması 28.4±12.8,
kadınların yaş ortalaması 32.5±13.0 olarak saptandı. Soygeçmişte konversiyon bozukluğu tanısı en sık görülen psikiyatrik tanı
olarak bulundu. Eşlik eden psikiyatrik tanılara bakıldığında hastaların %24.2’sinde duygudurum bozukluğu saptandı. Birincil
destek grubuyla olan sorunlar %51.1 ile en sık rastlanan stres etkeni grubu olarak belirlendi. En sık görülen belirtiler bayılma
(%59.2), hissizlik-uyuşma (%39.2), kasılma (%16.3), globus histerikus (%11.9) ve titreme (%10.6) olarak bulundu. Tartışma:
Karışık görünüm sergileyen alt tip %39.2 ile en sık görülen alttip idi. Bayılma ve duyu kaybı belirtileri motor bozukluk
belirtilerinden daha sık gözlendi.
Sociodemographic and clinical features of outpatients with conversion disorder in the psychiatry department of a university hospital
Objective: In this study, it is aimed to investigate sociodemographic and clinical features in outpatients with conversion
disorder who applied to the psychiatry department of an university hospital. Method: The study was carried out in outpatient
clinic at Psychiatry Department of Celal Bayar University Hospital between February 1995 and September 2000. The polyclinic
cards of 227 outpatients whose diagnosed as conversion disorder according to DSM-IV diagnostic criteria. Results: 88.1% of
the patients were women. 39.2% of the patients were in range of 15-24 [year]s. 52.4% of the patients were graduated from
primary school. 61.2% of the patients were housewife. 59% of the patients lived in the urban area. 64.8% of the patients were
married. The median age of men was 28.4±12.8; the median age of women was 32.5±13.0. Conversion disorder was the most
frequent mental disorder in the family history. In 24.2% of the patients, comorbid axis-I diagnoses were mood disorders.
51.1% of the patients had a stressor of the primary support group. The most frequent symptoms of conversion disorder were
seizures (59.5%), anesthesia-paresthesia (39.2%), contraction (16.3%), globus hystericus (11.9%), and tremor (10.6%).
Conclusion: The most frequent subtype of conversion disorder was mixed symptom type (39.2%). Symptoms of convulsion
and sensory loss were more frequent from symptoms of motor disorder.
