Londra, New York, Nantwitch ve Liverpool’da yürütülen bir dizi çalışmada 65 yaşın üzerindeki kişilerdeki depresyonların prevalansı ve başka epidemiyolojik özellikleri, doğal gidişi ve sonuçları araştırıldı. Bu çalışmalarda Geriyatrik Mental Durum Çizelgesi’nin topluma uyarlanmış şekli, Öykü ve Etiyoloji Ölçeği ve bilgisayarlı bir tanı sistemi kullanıldı. Normal yaşlılar beklenenin aksine yaşantılarından hoşnut ve oldukça aktiftiler. Yaşlılarda da önemli oranlarda depresyon görülüyor fakat yaşla birlikte depresyona yatkınlık artmıyordu. Depresyonu olan yaşlılarda tedavi görenlerin oranı düşük, mortalite oranı yüksekti. Spontan iyileşme olabiliyor, fakat bu iyileşme sıklıkla uzun zaman alıyordu. Depresyonu olan yaşlı hastaların önemli bir kısmına tanı konamamaktadır. Bu, hastaların çökkün duygudurumdan daha az yakınmaları, hastalıklarını bedenselleştirme ve depresyonu reddetme eğilimleri ya da yaşlıların doğal olarak çökkün olacakları inanışına bağlı olabilir. The authors investigated the prevalence and some other epidemiological aspects, natural course and outcome of depression in people over 65 [year]s old in their studies in London, New York, Nantwitch and Liverpool. They used the community version of Geriatric Mental State, History and Aetiological Schedule and a computer assisted diagnostic system. The results showed that the majority of normal old people were satisfied with their health and lives. Prevalence of depression did not seem to incera with age but still old people showed considerable levels of depressive pathology in varying severity. Mortality rates were higher in this group. Only a small group of depressed patients were found to be receiving specific treatment. Although most of the cases recovered spontaneously, this took a long time. Majority of depressions of old age are not diagnosed. Tendency of older people to deny depression, to somatize their illness, to complain less about depressed mood and to look less depressed and the belief that older people are naturally depressed may be the causes of under recognition. Prognoses of these depressions are not poor. The authors conclude that these patients should be recognized with proper evaluation and given a chance by administering specific treatment.
