Bu çalışmada bir tıp fakültesinin genel dahiliye polikliniğinde psikotrop ilaç ve özellikle antidepresan kullanımının sıklığı ve niteliği aynı fakültenin psikiyatri polikliniğindeki uygulamayla karşılaştırılarak araştırılmıştır. Dahiliye polikliniğine ilk kez başvuran hastaların yaklaşık beşte birine bir antidepresan başlanmasına karşın depresyon tanısının hiç kullanılmadığı, bu hastaların çoğuna ya hiçbir psikiyatrik tanı konmadığı ya da genel olarak “psikonevroz” tanısı konduğu, önerilen ilaç dozlarının hemen her olguda yetersiz olduğu ve hastaların belli bir izleme programına alınmadığı görülmüştür. Psikiyatri polikliniğinde ise ayaktan tedavinin temel ilkelerine uyulduğu saptanmıştır. Psikiyatri klinikleri dışında antidepresanların doğru kullanılabilmesi için eğitim ve konsültasyon-liyezon etkinliklerinin ilerletilmesi gerekmektedir.

In this study the frequency and the quality of the use of antidepressants at the general internal medicine outpatient clinic of a university hospital have been investigated and discussed in comparison with the relevant outpatient practice at the psychiatry department of the same hospital within a given period. Although an antidepressant drug was prescribed in about one-fifth of internal medicine outpatients, none of them were diagnosed as having depression. In the majority of the patients either no psychiatric diagnosis was given or the category of “psychoneurosis”, which seems to cover a wide range of clinical disorders, was employed. The recommended dosage was subtherapeutic in almost every case and in almost none of the patients a follow-up interview was arranged. The comparable practice at the psychiatry department was characterized with a general adherence to the basic principles of outpatient pharmacotherapy. Our results indicate an obvious need for improvement of a better use of pharmacotherapy in depressed patients outside psychiatry clinics.
