Santral serotonin (5HT) sendromu, monoamino oksidaz (MAO) inhibitorleri ile santral serotonin düzeyini arttıran ilaçların birlikte kullanılması sonucu ortaya çıkan bir sendromdur. İlk olarak 1950 yıllarında bu ilaçları birlikte kullanan hastalarda sendroma ait belirtiler gözlenmiş, fakat tanımlanamamıştır. İnsanlarda ilk defa 1982 yılında tanımlanan sendromun patofizyolojisinden santral sinir sisteminde serotonin düzey artışının ve postsinaptik 5HT1A reseptörlerinin uyarılmasının sorumlu olduğu kabul edilmektedir. Sendromun bulguları arasında ajitasyon, mental durum değişiklikleri, hiperrefleksi, tremor ve hiperpireksi sayılabilir. Serotonerjik ilacın kesilmesi ile belirtiler genellikle 24 saat içinde kaybolmaktadır. Son yıllarda serotonerjik antidepresan ilaçların klinik uygulanımının artması ve sendromun ölümcül olabilmesi nedeni ile bu derlemede sendromun oluşum mekanizması, tanı kriterleri ve tedavisi tartışılmıştır.

Central serotonin syndrome is most commonly the result of the interaction between serotnergic agents and monoamine oxidase inhibitors. Central Serotonin Syndrome symptoms had been observed in patients using these drug combinations since late 1950’s but the syndrome could not be defined until 1982. The presumed pathophysiological mechanism involves the activation of postsynaptic 5HT1A receptors which produces an increase in the central serotonin levels. The most frequent clinical features are agitation, alterations in the mental status, hyperreflexia, tremor and fever. The syndrome typically resolves within 24 hours with the discontinuation of the suspected serotonergic agent. Although Central Serotonin Syndrome is usually a self limited condition, early recognition and treatment is necessary because of the fatalities which may occur.
