Lityumun böbrek işlevlerine etkisi yaygın olarak araştırılmıştır. Glomerüler işlevler pek etkilenmezken, tübüler işlevlerde değişiklikler bildirilmiştir. Etkilerin patofizyolojisi hala yeterince aydınlatılamamıştır. Bu çalışmada lityumun böbrek işlevlerine etkisinin, oldukça az çalışılmış olan eritrosit lityum düzeyi ile ilişkisi araştırılmıştır. Çalışma, yatırılarak incelenen, bipolar bozukluk remisyonda tanılı 108 hasta ile gerçekleştirilmiştir. Olguların %14.8’inde (n=16) poliüri, %38.9’unda (n=42) konsantrasyon yetisinde bozulma (osmolalite düşüklüğü) belirlenmiştir. Poliüri lityum dozu, plazma ve eritrosit lityum düzeyleri ile, osmolalite düşüklüğü ise idrar B2-mikroglobulin (B2-Mg) ve eritrosit lityum düzeyi yüksekliği ile ilişkili bulunmuştur. Bu sonuçlar, tübüler protein geriemilimi, böbrek konsantrasyon yetisinde bozulma ve eritrosit lityum düzeyini belirleyen süreçlerde bir kesişmeyi düşündürmektedir.

Lithium’s effect on renal function is widely investigated. While effects on glomerular functions are minimal, changes in tubuler functions are reported. Satisfactory explanations of pathophysiology of this process are not yet available. In this study, an infrequently investigated aspect, relationship between lithium’s effect on renal functions and erythrocyte lithium level are investigated. We performed our study in an inpatient unit with 108 bipolar patients in remission. 14.9% (n=16) of the cases showed polyuria and 38.9% (n=42) showed disturbed concentration ability (decreased osmolality). We found a relationship between polyuria and lithium dosage, plasma and erythrocyte lithium level and also between decreased osmolality and inreased urine B2-microglobulin (B2-Mg) and erythrocyte lithium level. These results bear to mind an intersection in processes which determine tubular protein reabsorption, disturbance in the ability of concentration and erythrocyte lithium level.
