Amaç: Premenstrüel Disforik Bozukluk (PDB) menstrüel döngünün adet öncesi döneminde ortaya çıkan ve adet kanamasının başlamasıyla kaybolan, duygu ve davranış değişiklikleri ve fiziksel belirtilerle kendini gösteren bir bozukluktur. Kadınlar arasındaki yaygınlığı % 3-8 arasındadır. Bu çalışmanın amacı kadın hastalıkları ve doğum polikliniğinde premenstrüel sendrom tanısı konulan kadınların belirti örüntüsünü, adet öncesi ve sonrası dönemdeki belirti şiddetlerini araştırmaktır. Yöntem: DSM-IV kriterlerine göre PDB tanısını karşılayan 28 kadın çalışmaya alınmıştır. Adet öncesi ve sonrası dönemde Beck Depresyon Envanteri Durumluk Kaygı Ölçeği (DKÖ) ve belirti tarama listesi uygulanmıştır. Sonuç: Uygulanan ölçekler sonucunda kadınlarda adet öncesi dönemde ruhsal belirtilerin daha sık ortaya çıktığı ve bu belirtilerin şiddetinin adet öncesi dönemde adet sonrası döneme göre anlamlı düzeyde yüksek olduğu saptanmıştır. Bedensel belirtilerde ise anlamlı düzeyde değişiklik gözlenmemiştir. Tartışma: Bu bulgular doğrultusunda PDB’de psikiyatrik belirtilerin belirleyici olduğu ve bu nedenle psikiyatri tarafından ele alınmasının uygun olacağı kanısına varılmıştır.

Objective: Premenstrual dysphoric disorder (PDD) is characterized by emotional, behavioral, and physical symptoms that occur in the premenstrual phase of the menstrual cycle, with resolution after menses. Its prevalence is %3-8. The aim of this study is to investigate the symptomatology during the premenstrual period and the change in the severity of the symptoms in premenstrual and postmenstrual periods among women who had the diagnosis of premenstrual syndrome in gynecology outpatient clinic. Method: The study group was consisted of 28 women who had the diagnosis of PDD according to DSM-IV criteria. Beck Depression Inventory, State Anxiety Inventory and a symptom check list were applied during the premenstrual and postmenstrual period. Results: The scales used in the study revealed that in the premenstrual phase psychological symptoms are more common and the severity of these symptoms is significantly higher in the premenstrual phase compared to the postmenstrual phase while no differences were found in somatic symptoms. Conclusion: In the light of these results, it can be concluded that psychiatric symptoms are diagnostic for PDD and it should be managed by psychiatrists.
