Çocuk ve ergenlerdeki intihar davranışları tüm dünyada major bir halk sağlığı sorunudur. Klinik ve toplum temelli çalışmaların sonuçları intihar davranışının ergenlik döneminde sık görüldüğünü göstermekte ve gelişimsel açıdan değerlendirilmesi gerektiğine işaret etmektedir. Ergenlerdeki intihar davranışları, erişkinlerden farklı özellikler taşır ve intihara eğilimli ergenlerin tedavisinde gelişimsel değerlendirmeler önem taşır. Ergen intihar davranışı genellikle aile içi çatışmaların olduğu, akademik ve disiplin alanında zorlukların görüldüğü durumlarda, ve/veya ergenin büyüdükçe daha da önem verdiği akran, romantik arkadaş ilişkilerinde bozulmaların sonucunda görülmektedir. Çocuk ve ergenlerde görülen intihar davranışlarına yönelik hali hazırda güvenilir, etkin psikoterapi ya da psikofarmakolojik müdahale ve tedaviler bulunmamaktadır. İntihara eğilimli ergenlerin tedavisinde verilerin yetersiz olması nedeniyle bir müdahalenin bir diğerine üstün olup olmadığını bilemiyoruz. Bu gruptaki heterojenite nedeniyle “herkes için geçerli” etkin bir tedavi yaklaşımı bulunmamaktadır. Müdahale ergenin ihtiyacına göre tasarlanmalıdır. Duygusal açıdan ulaşılabilir ve sağlıklı bir aileye sahip olan, akranlarıyla iyi ilişkileri ve sosyal desteği bulunan, geleceğe dair umut besleyen, çatışmalarını çözmek için gayreti olan ergenlerde kısa süreli krize müdahale gerekli iken, daha önce intihar girişimlerinde bulunan, yoğun intihar düşünceleri olan, ciddi depresyon veya psikiyatrik bozukluğa sahip, alkol ya da madde kullanımı olan ve işbirliğine yanaşmayan ailelere sahip ergenlerde psikiyatrik yatış gerekli olmaktadır. Bu yazıda, literatür ışığında, çocuk ve ergenlerde görülen intihar davranışında, değerlendirme, belirlenmiş risk etmenleri, koruyucu etmenler, kriz yönetimi, gelişimsel hususlar, psikoterapiler ve psikofarmakolojik müdahale ve tedavilerin gözden geçirilmesi sunulmuştur.

Suicidal behavior in children and adolescents is a major health problem in all over the world. Both the clinical and community-based study results show that suicidal behavior is common during adolescence and needs to be considered in a developmental context. Suicidal behavior of adolescents occurs in different contexts than the suicidal behavior of adults. The development of a treatment for suicidal behavior in youths presents many challenges. Developmental considerations are important in the treatment of suicidal adolescents. Adolescent suicidal behavior often occurs in the context of family conflict, academic and disciplinary difficulties, or as a consequence of distortions in peer, partner relationships that are increasing in importance as adolescents get older. Despite progress in the identification of risk factors for suicidal behavior, there are no effective, reliable psychotherapeutic or psychopharmacological interventions that have been shown to reduce the suicidal behaviors in children and adolescents. There are insufficient data available to recommend one intervention over another for the treatment of suicidal adolescents but interventions that have potential for greater effectiveness in reducing adolescent suicidal behavior are discussed in this review. Given the heterogeneity among adolescent suicide attempters, it is unlikely that a “one size fits all” approach to treatment will testify effective for suicidal adolescents. Intervention should be tailored to the adolescent’s needs. Adolescents with an emotional available intact family, good peer relations and social support, hope for the future, and a motivation to work out conflicts may require only brief crisisoriented intervention but adolescents who have made previous suicide attempts, exhibit a high degree of intent to commit suicide, have serious depression or other psychiatric illness, are abusing alcohol or drugs, and have families who are unwilling to commit to counseling are at high risk and may require psychiatric hospitalization. In the light of the literature this review provides an overview of assessment, identified risk factors, protective factors, crisis management, developmental issues, psychotherapeutic and psychopharmacological interventions and treatments for suicidal behavior in children and adolescents.
