Demans, merkezi sinir sisteminin çeşitli hastalıklarından köken alan genel bir sendromdur. Lewy cisimcikli demans, demansın bir alt tipidir ve beyin hücrelerine zarar veren mikroskobik depozitlerden dolayı, yürütücü fonksiyonlarda bozulmaya sebep olmaktadır. Lewy cisimcikleri aynı zamanda Alzheimer Hastalığı ve Parkinson Hastalığı'na bağlı demansta da bulunmaktadır. Parkinson hastalarında zamanla kognitif fonksiyonlarla ilgili problemler gözlenebilir aynı zamanda Lewy cisimcikli demanslı hastalarda eğik postür, kaslarda rijidite, harekete başlamada zorluk gibi hareket semptomları görülebilir. Semptomlardaki bu benzerlik Lewy cisimcikli demans ve Parkinson hastalığına bağlı demansın altta yatan sebebinin ortak olabileceği düşünülmektedir. Bu konuda alfasynuclein proteinin işlenmesindeki bozulma hipotezi sorumlu tutulmaktadır. Bu derleme yazısında, Lewy cisimcikli demansın klinik bulguları, ayırıcı tanısı, prognozu ve tedavisi tartışılmıştır.

Dementia is a general syndrome that can be caused by a various diseases of central nervous system. Dementia with Lewy bodies is a subtype of dementia that leads to a decline in executive fuctions because of abnormal microscopic deposits that devastate brain cells. Lewy bodies are also found in Alzheimer's disease and Parkinson's disease dementia. Many patient with Parkinson's disease eventually develop problems with cognitive functions, and many patient with Dementia with Lewy bodies experience movement symptoms, such as humpy posture, rigid muscles and trouble initiating movement. This overlap in symptoms suggest that Dementia with Lewy bodies and Parkinson's disease dementia may be linked to the same underlying defect in how the brain processes the protein alpha-synuclein. In this review we discuss the clinical signs, differential diagnosis, prognosis and treatment of Dementia with Lewy bodies.
