Konversiyon Bozukluğundaki Ruhsal Bozukluk Eştanılarının Sosyodemografik Değişkenlerle İlişkisi

Amaç: Konversiyon hastalarının özellikleri konusunda çok az çalışma yapılmıştır. Bu çalışmamızın amacı konversiyon bozukluğu olan hastalarda saptanan şimdiki eksen-I ruhsal bozuklukların klinik, demografik ve psikososyal değişkenlerle ilişkili olup olmadıklarını belirlemektir.
Yöntem: DSM-III-R konversiyon bozukluğu tanılı 67 hastaya SCID-I ve SCID-II uygulanarak ruhsal bozukluk eştanıları elde edilmiştir. Demografik veriler, klinik özellikler ve belirti alttipi gibi değişkenlerin şimdiki eksen-I bozukluklarıyla olası ilişkileri, değişkenlerin relatif riskleri saptanarak araştırılmıştır.
Bulgular: En sık görülen konversiyon bozukluğu alttipleri nöbet (%40.3) ve karışık (%34.3) belirtili alttiplerdir. Ayrıca karışık belirtili alttipin en kronik konversiyon türü olduğu anlaşılmıştır. Son konversiyon episodunda saptanabilen stressör oranı %67.2 olup, hastaların %73.1’i geçmişlerinde ağır ya da travmatik stresörler tanımlamışlardır. Evli hastalarda herhangi bir affektif bozukluk tanısı ise 2.8 kat daha fazla görülmekteydi. 17-24 yaş grubunda dissosiyatif bozukluklar, yaşlı grupta ise affektif bozukluklar daha fazla eşlik etmekteydi. Karışık alttipte affektif bozukluk, duyusal tipte ise anksiyete bozukluğu relatif riskleri anlamlı olarak daha yüksek bulunmuştur.
Sonuç: Konversiyon bozukluğundaki eştanı dağılımı farklı etiyolojik etkenlerin konversiyon belirtilerinin ortaya çıkışında ve sürmesinde önemli olabileceğini düşündürmüştür.
The Relationship of Comorbidity in Conversion Disorder to Sociodemographic Variables
Objective: There is little empirical research about the characteristics of conversion patients. The purpose of this study was to determine whether the frequencies of any current axis-I psychiatric diagnoses in conversion disorder patients are associated with clinical, demographic and some psychosocial variables.
Method: Consecutively referred 67 outpatients with DSM-III-R conversion disorder were examined with SCID-I and SCID-II to assess comorbid mental disorders. The effects of some variables such as demographic data, clinical features and subtype status, to the current axis I diagnoses were also investigated by calculating the relative risks.
Results: The most prevalent subtypes of conversion disorder were convulsive symptom type (40.3%) and mixed symptom type (34.3%). Mixed symptom group appeared as the most chronic form of the conversion disorder subtypes. The last episode of conversion episode was related with a stressor in 67.2% of the patients and 73.1% described severe or traumatic past stressors. Among the studied variables in conversion patients, the comorbidity of any axis-I disorder and comorbidity of affective disorders were seen 7.4 and 2.8 times more in married patients than single patients respectively. Dissociative disorders were significantly more prevalent in 17-24 [year] patient group, whereas older conversion patients were suffering more from comorbid affective disorders. Relative risk of having an affective disorder were higher in mixed subtype of conversion disorder and also relative risk of anxiety disorders were high in subtype with sensory symptoms.
Conclusion: The comorbidity pattern in conversion disorder reflects different etiological factors contributing to the onset and endurance of conversion symptoms.
