Sosyal Fobide Klinik Özellikler ve Eştanı

Amaç: Sosyal fobi, erken yaşta başlayan, kronik gidişli bir anksiyete bozukluğu olup, sık rastlanan eştanılı durumlar bireyde eğitim, toplumsal ve mesleki alana belirgin işlev kayıplarına neden olur. Bu çalışmanın amacı, klinik bir örneklemde sosyal fobinin kilinik özelliklerin ve eştanılı durumların sıklığını araştırmaktır.
Yöntem: Bakırköy Ruh ve Sinir Hastalıkları Hastanesi 2. Nevroz Polikliniğin başvuran ve DSM-IV ölçütlerin göre sosyal fobi tanısı alan 44 hastaya yarı yapılandırılmış sosyodemografik veri formu, SCID-OP, SCID-II, Hamilton Deprasyon ve Hamilton Anksiyete Değerlendirme Ölçekleri ve Liebowitz Sosyal Anksiyete Ölçeği uygulandı.
Bulgular: Örneklemin 36’sı (%82) erkek, 8’i (%18) kadındı. Yaş aralığı 18-58 olup, ortalaması 27.34’tü. Grubun %52.3’ünde eşlik eden I. eksen tanısı saptanırken, bunların %33’ü agorafobili panik bozukluğu idi. Eştanılı durumların %76’sında sosyal fobi, eştanıdan önce başlamaktaydı. Hastaların %68.2’sinde II. eksen tanısı vardı ve bunların %50’si çekingen kişilik bozukluğuydu.
Sonuç: Bu çalışmada eştanı sıklığı literatürdeki çalışmaların büyük çoğunluğundan daha düşük bulunmuştur. Çekingen kişilik bozukluğu ile sosyal fobi eştanısı ayrıca araştırılması ve tartışılması gereken bir konu olarak karşımıza çıkmıştır.
Clinical Features and Comorbidity in Social Phobia
Objective: Social phobia, as a chronic disorder that has an early onset, and having a high frequencey of comorbid disorders, causes considerable impairment in educational, vocational and social functions. The aim of this study is to investigate the cilinical features and frequency of related comorbid disorders of social phobia in an outpatient sample.
Method: The study has been conducted among patients referred to the outpatient department of Bakirköy State Hospital for Psychiatric and neurologic Disases-Neurosis Unit, 44 subjects who met DSM-IV criteria for social phobia were selected for investigation. The subjects were assessed by using a semi-structured sociodemographic form, SCID-OP, SCID-II, Hamilton Rating Scale for Depression, Hamilton Rating Scale for Anxiety and Liebowitz Social Anxiety Scale.
Results: 36 (%82) of the group were male, 8 (%18) were female. The ages varied between 18-58, with a mean of 27.3 . 52.3% of the subjects were assessed as having a comorbid axis I disorder of which 33% consisted of panic disorder with agoraphobia. In the 76% of cases the onset of social phobic symptoms preceded the symptoms of comorbid disorder. An additional axis II disorder was found in 68.2% of the subjects, 50% of them were diagnosed as having avoidant personality disorder.
Conclusion: The frequency of comorbid disorders in our social phobic sample is lower than most of the studies in the literature. The interface between social phobia nad avoidant personality disorder needs to be studied and discussed further.
