Bu makalede 15 yaşında karotis arterleri üzerine bası yaparak senkop nöbetleri yaratan ve bundan haz alan bir olgu sunulmaktadır. Bu davranış başlangıçta arkadaşlar arasında yapılan ve keyif alınan bir nitelikteyken sonraları sıklığı giderek artmış, neredeyse vazgeçilemez bir hal almıştır. Başvuru yapıldığında ise olgu bu davranıştan kurtulmak için çare arar durumdaydı. Bu olgu, DSM-IV tanı sistemi penceresinden bakıldığında ?başka türlü sınıflandırılmamış dürtü kontrolü bozuklukları? içinde sınıflandırılabilir. Ancak, obsesif kompulsif spektrum bozukluklarının kompulsivite/impulsivite boyutunda bir ara noktaya yerleştirmek ya da dışarıdan alınan herhangi bir madde olmamasına karşın, ortaya çıkan psikoaktif etki ve fizyolojik değişiklikler gözönünde bulundurulacak olursa bağımlılıkla ilişkilendirmenin de olası olabileceği görülmektedir.

In this article a 15 [year] old boy who derives satisfaction by self induced syncope is presented. Syncope occurs when the patient exerts pressure on carotid arteries on two sides. This behavior started as a pleasurable activity shared with a group of friends. But by time, the frequency increased and it became almost impossible to quit. At the time of attendance the boy was looking for help in order to get rid of this symptom. This case can be classified as ?impulse control disorder not otherwise specified? according to DSM-IV diagnostic system. But the same behavior can also be viewed in terms of compulsivity and impulsivity dimension of obsessive compulsive spectrum disorders. Moreover, even though there is no substance intake, because of the psychoactive effect of the symptom and the physiological changes which follow, it can be viewed as a substance dependence as well. This case points out the need for a broader point of view in describing and analyzing human behavior.
