Majör Depresif Bozuklukta Tiroid İşlevlerinin İncelenmesini Etkileyen Faktörler

Amaç: Majör depresif bozuklukta tiroid işlev incelemesi için seçilen hastaların özelliklerinin değerlendirilmesi ve bu grupta tiroid işlev incelemesinin gerekliliğinin belirlenmesi.
Yöntem: Psikiyatri polikliniğine bir yıl boyunca ayaktan başvuran ve majör depresif bozukluk, tek epizod tanısı konan 164 hasta çalışmaya alınmıştır. Bu hastaların poliklinik kayıtları geriye dönük olarak değerlendirilmiş ve sosyodemografik özelliklerin yanısıra depresif belirtiler değerlendirmeye alınmıştır. Tiroid işlev incelemesinin hangi hastalardan isteneceği hekimin kendi klinik izlenimi ve deneyimine göre belirlenmiştir.
Bulgular: Hastalardan %65.2'si ne tiroid işlev incelemesi istenmemiş, %34.8'i ne istenmiştir. Bu gruplar sosyodemografik özellikler yönünden karşılaştırıldığında, eğitim görmemişlerde anlamlı olarak daha fazla inceleme istenmiştir. Diğer özellikler farklılık yaratmamaktadır. Depresif belirtiler yönünden karşılaştırıldığında ise, ?suçluluk duygusu? tiroid işlev incelemesi istenmeyen hastaları istenenlerden ayırt etmektedir. Tiroid işlev incelemesi istenen hastalardan birinde subklinik hipotiroidi, birinde subklinik hipertiroidi saptanmıştır.
Sonuç: Majör depresif bozukluğun tanısal değerlendirmesine tiroid işlev incelemesinin pek katkısı yoktur. Ayrıca, hastanın kendisini ifade edememesi gibi sebeplerle, psikiyatristin tanısal değerlendirmede yetersizlik hissettiği durumlarda ek incelemeye başvurulmaktadır.
Clinical Factors Affecting Thyroid Screening in Major Depressive Disorder
Objective: We aimed at assessing the selection criteria for thyroid screening in patients with major depressive disorder and determining the need for screening thyroid dysfunction in this group.
Method: 164 patients with major depressive disorder, single episode, attending the outpatient clinic of psychiatry during one [year] were included in the study. The records of the patients were reviewed retrospectively regarding both sociodemographic features and depressive symptomatology. Psychiatrists have chosen the thyroid screening method according to their clinical impression and experience.
Results: 65.2% of the patients were not required any thyroid screening, whereas in 34.8% of the patients thyroid function tests were performed. When these two groups were compared in terms of their demographic features, statistically more tests were required for illiterate patients. Other demographic features did not differ among the two groups. In terms of depressive symptoms, ?feelings of guilt? was discriminating the patients without thyroid screening from the patients with thyroid function tests. Among the patients who had thyroid function tests, only one patient had subclinical hypothyroidism and one patient had subclinical hyperthyroidism.
Conclusion: Thyroid screening may add little to the diagnostic evaluation of major depressive disorder. Moreover, psychiatrists request further tests only when they feel uncomfortable about diagnosis.
