Manisa il merkezinde doğum sonrası depresyon yaygınlığı ve etkileyen etmenler

Amaç: Kadın yaşamının bazı dönemlerinde depresyon riski artmaktadır, doğum sonrası dönem de bu riskin arttığı dönemlerden birisidir. Ülkemizde, doğum sonrası depresyonun yaygınlığı ve bunu etkileyen etmenler ile ilgili net bilgilere rastlanmamaktadır. Bu çalışmanın amacı Manisa il merkezinde doğum sonrası depresyonun yaygınlığını ve bunu etkileyen etmenleri araştırmaktır.
Yöntem: Bu amaçla Manisa il merkezine bağlı toplam 10 sağlık ocağında 0-6 aylık bebeği olan 1337 anneden rastgele örnekleme yöntemi kullanılarak 317’si seçilmiş ve bunların 257’sine (%81,7) ulaşılmıştır. Veriler Edinburgh Doğum Sonrası Depresyon Ölçeği ve bu çalışma için hazırlanmış sosyodemografik anket formu kullanılarak toplanmıştır.
Bulgular: Depresyon puan ortalaması 7,54±4,66 olarak saptanmıştır. Kesme noktası 13 alındığında ise %14’ünün depresyon tanısı aldığı görülmüştür. Yaşayan çocuk sayısının fazla olması, gecekonduda yaşama, göç etme, bebeğinde önemli sağlık sorunu olması daha önce psikiyatrik bir bozukluğu olması, eşinde psikiyatrik bozukluk olması, eşiyle ve eşinin ailesiyle ilişkisinin iyi olmaması da depresyon ortaya çıkışıyla ilişkili etmenler olarak saptanmıştır.
Sonuç: Manisa il merkezinde, postpartum depresyonun önemli bir sağlık sorunu olduğu, sosyoekonomik düzeyi düşük olan, daha önce psikiyatrik rahatsızlık geçiren ve sosyal ilişkileri yeterli olmayan kişilerin postpartum dönemde erken tanı ve tedavi için daha dikkatli izlenmesi gerektiği sonucuna varılmıştır.
The Prevalence of Postpartum Depression in Manisa Province and Influencing Factors
Objective: In some periods of the woman’s life the risk of depression increases; the postpartum period is one of these periods. There is no clear information about the prevalence and the factors affecting postpartum depression in this country. The aim of this study was to investigate the prevalence and the factors influencing postpartum depression in Manisa province.
Method: There were 1337 women who were recorded as having babies aged between 0 and 6 [month]s in 10 primary health care centers in Manisa province. A sample group of 317 mothers was randomized among these and 257 (81.7%) of the sample group could be contacted. Data was collected by using the Edinburgh Postpartum Depression Scale and a questionnaire on sociodemographic variables, which was designed for this study.
Results: The mean depression score was found to be 7.54±4.66 when a cut-off point of 13 was used, 14% of the mothers were at risk for depression. The factors which affected the prevalence of depression were as follows: number of living children, living in a shanty, being a migrant, serious health problems in the baby, history of psychiatric problems, psychiatric disorder in the spouse, and having bad relations with the spouse and his parents.
Conclusion: It was concluded that postpartum depression is a significant health problem in Manisa province, and that individuals of low socioeconomic level and those who suffered from psychiatric illness before and those who have insufficient social interaction should be followed up with more care during the postpartum period for the sake of early diagnosis and treatment.
