Depresyon hastalarında bilişsel işlevler

Amaç: Araştırmanın amacı depresyonun bilişsel işlevler üzerine etkilerini araştırmaktır.
Yöntem: 15’i kadın ve 10’u erkek olmak üzere toplam 25 hasta ile, yaş, cinsiyet ve eğitim düzeyleri bakımından hasta grubu ile eşleştirilmiş 25 sağlıklı denekten oluşan kontrol grubu araştırmaya alınmıştır. Hasta ve kontrol gruplarına, Sözel Bellek Süreçleri Testi (SBST), Benton Görsel Bellek Testi (BGBT), Kısa Kognitif Muayene (KKM), İşaretleme Testi (İT), Renk İzleme Testi (RİT) ve Sözel Akıcılık Testi (SAT) uygulanmıştır. Hasta grubunun depresyon ve anksiyete düzeyleri Hamilton Depresyon (HDÖ) ve Anksiyete (HAÖ) ölçekleri ile değerlendirilmiştir.
Bulgular: Kullanılan testlerden SBST ve KKM testlerindeki başarıları bakımından hasta ve kontrol grupları arasında bir farklılık bulunmamıştır. BGBT, SAT, İT ve RİT testlerinde ise gruplar arasında bazı anlamlı farklılıklar bulunmuştur. Hasta grubunda bilişsel işlevlerde saptanan bozukluklar ile HDÖ puanları arasındaki bağıntı anlamlı bulunmuştur.
Sonuç: Depresyon hastalarında görsel bellek, görsel-motor izleme, odaklanmış dikkat ve sözel akıcılık becerilerinde bozukluklar saptanmıştır. Bu bozuklukların hastaların depresyonlarının şiddetleri ile bağıntılı olarak artış gösterdiği görülmüştür.
Cognitive Functions in Depressed Patients
Objective: This study was designed to ascertain the degree and specifity of cognitive impairments in patients with depression.
Method: Twenty-five depressed patients and twenty-five control subjects of comparable age and education level were evaluated. Cognitive functions were assessed with a neuropsychological test battery which consisted of the Rey Auditory-Verbal Learning Test (RAVLT), Benton Visual Memory Test (BVMT), Brief Cognitive Examination Scale (BCES), Cancellation Test (CT), Color Trails (CoT) and Verbal Fluency Test (VF). The severity of depression and anxiety in the patient group were evaluated using the Hamilton Anxiety and Depression Scales.
Results: The performance of the depressed group was significantly below that of the control group in terms of the BVMT, VF, CT and CoT. In terms of the other measures, there was no meaningful difference between the groups. Statistical analysis showed that there is a significant correlation between the neuropsychological impairment and the severity of depression.
Conclusions: The results indicated that cognitive functions such as visual memory, visual-motor tracking, focused attention and verbal fluency were impaired in depressed patients and the impairment of these functions correlated with the severity of depression.
