Obsesif kompulsif bozukluğu olan hastalarda kantitatif EEG

Amaç: Bu çalışmada, obsesif kompulsif bozukluğu (OKB) olan hastalarda beynin elektrofizyolojik değişimlerinin kantitatif-EEG (K-EEG) yöntemi ile incelenmesi ve bu değişikliklerle klinik özellikler arasındaki ilişkinin araştırılması amaçlanmıştır.
Yöntem: Çalışma, DSM-IV ölçütlerine göre OKB tanısı konulan 32 hasta yaş ve cinsiyet açısından hasta grubuna benzer 31 sağlıklı kontrol deneği ile yürütülmüştür. Çalışmaya alınan tüm bireylerin sosyodemografik verileri kaydedilmiş ve Maudsley Obsesif Kompulsif Soru Listesi, Hamilton Depresyon Derecelendirme Ölçeği verilmiştir. Tüm deneklere bilgisayarlı EEG kaydı uygulanmıştır. Hasta grubunda obsesif kompulsif bozukluğun şiddeti Yale-Brown Obsesif Kompulsif Ölçeği ile değerlendirilmiştir.
Bulgular: Hasta grubunda temporal bölgede, mutlak beta frekans bant gücünde azalma, delta ve teta frekans bant gücünde artma, sol frontopariyetal bölgede ise oransal beta frekans bant gücünde azalma saptandı. Kontrol ve hasta grubu arasında saptanan bu farklar istatistiksel olarak anlamlı düzeydeydi. Kontrol etme kompulsiyonu olan hastalarda pariyeto-oksipital bölgede alfa ve beta bant gücü azalması ve kortekste yaygın olarak teta bant gücü artışı belirlendi. Eşlik eden depresyonun alfa frekans bant gücünü etkilediği görüldü.
Sonuç: K-EEG yönteminin sağladığı bulgular, heterojen özelliklere sahip obsesif kompulsif bozukluğun klinik tanısı ve klinik alt tiplerinin belirlenmesinde yararlı olabilir. Ancak K-EEG yöntemi ile saptanan elektrofizyolojik değişikliklerin bu hastalık için özgül olduğunu söyleyebilmek için benzer başka çalışmalara gerek vardır.
Quantitative EEG in Patients with Obsessive Compulsive Disorder
Objective: The aim of this study was the investigation of electrophysiological changes in the brain by quantitative EEG (Q-EEG) in subjects with obsessive compulsive disorder (OCD) and the relationship between these changes and clinical features.
Method: The study was performed on 32 patients diagnosed with OCD according to DSM-IV criteria, and 31 healthy subjects similar to the OCD group in terms of age and sex. Sociodemographic data of the subjects included in the study were recorded, and subjects were administered the Maudsley Obsessive Compulsive Questionnaire and the Hamilton Depression Rating Scale. Computerized EEG was recorded for all subjects. The severity of OCD in the patient group was evaluated by the Yale-Brown Obsessive Compulsive Scale.
Results: An absolute beta frequency band power decrease, and absolute delta and theta frequency band power increases were determined in the temporal region, and a relative beta frequency band power decrease in left frontoparietal region in the patient group. The differences between two groups in these parameters were statistically significant. Alpha and beta frequency band power decreases were determined in the parieto-occipital region, and a generalized theta frequency band power increase was determined in the cortex of patients with checking compulsion. It was observed that comorbid depression affected alpha frequency band power.
Conclusion: Findings obtained by quantitative EEG method may be helpful in the diagnosis of heterogenous OCD and the identification of clinical subgroups. However, additional investigations are necessary to determine whether the electrophysiological changes determined are specific to this disorder.
