Agorafobili ve agorafobisiz panik bozukluğu: Hipotalamo-Pituiter-Tiroid eksen bulguları ve otonomik işlevler açısından bir karşılaştırma

Amaç: Bu çalışma, agorafobili ve agorafobisiz panik bozukluğu (PB) olan olguların hipotalamo-pitüiter-tiroid (HPT) eksen bulgularının ve bazı otonomik işlevlerinin karşılaştırılması amacıyla yapılmıştır.
Yöntem: Çalışmaya, DSM-IV ölçütlerine göre tanı konan ve ardışık olarak seçilen 14 adet agorafobili ve 16 adet agorafobisiz panik bozuklukluğu olan hasta ile 24 sağlıklı denek alınmıştır. HPT eksen bulgularını değerlendirmek için periferik tiroid hormonları (ST3, ST4, TT3, TT4), TRH’a TSH yanıtları ve Dmax TSH düzeyleri; otonomik işlevleri değerlendirmek için ise TRH infüzyonu sonucu oluşan arteriyal tansiyon ve nabız değişiklikleri kullanılmıştır.
Bulgular: Normal sınırlarda kalsa da, agorafobili ve agorafobisiz PB’lu olguların ortalama periferik tiroid hormon düzeyleri sağlıklı gruba göre anlamlı olarak yüksek bulunmuştur. Hasta grupları ile sağlıklı grup arasında TRH’a TSH yanıtları açısından anlamlı bir farklılık elde edilememiştir. Agorafobili ve agorafobisiz hastalarda TRH’a TSH yanıtlarındaki körelme oranı sırasıyla %28.5 ve %31.5 bulunmuş ve bu oranlar sağlıklı grubun körelme oranından (%0) anlamlı olarak yüksek bulunmuştur. Hasta ve sağlıklı grupların TRH infüzyonuna bağlı oluşan otonomik yanıtları benzer bulunmuştur.
Sonuç: Bu bulgulara dayanarak, gerek HPT eksen bulgularının gerekse TRH’a gösterilen otonomik yanıtlarının agorafobili ve agorafobisiz panik bozukluğu olan olgular arasında belirgin farklılıklar göstermediği bu iki panik bozukluğu tipinin en azından HPT eksen ve otonomik bulgular açısından benzer fizyopatolojik mekanizmalarla ilişkili olabileceği söylenebilir.
Panic Disorder with and without Agoraphobia: A Comparative Study with respect to the Hypothalamo-Pituitary-Thyroid Axis and Autonomic Functions
Objective: This study was designed to compare hypothalamo-pituitary-thyroid (HPT) axis findings and certain autonomic functions in panic disorder (PD) patients with and without agoraphobia.
Method: According to DSM-IV criteria, 14 PD patients with agoraphobia and 16 without agoraphobia who were consecutively selected, and 24 healthy control subjects were included in the study. Peripheral thyroid hormones (free and total T3 and T4 levels), TSH responses to TRH and Dmax TSH values were used to evaluate HPT axis functions; and arterial blood pressure and pulse changes of the patients due to TRH infusion were used to assess autonomic functions.
Results: Although within normal ranges, the mean peripheral thyroid hormone levels of the PD patients with and without agoraphobia were significantly higher than those of the healthy controls. No significant differences were found between the PD patient groups and the control group for TSH responses to TRH. The rates of blunted TSH responses to TRH in PD patients with and without agoraphobia were 28.5% and 31.5%, respectively, and these rates were significantly higher than the rate of controls (0%). Autonomic responses and due to TRH infusion were similar in all groups.
Conclusions: These findings show that no significant diffreneces were found between PD patients with and without agrophobiia with regard to HPT axis functions and autonomic responses to TRH, and suggest that the mechanisms related to the physiopathology of PD and its subtypes may also be similar, at least in terms of the HPT axis and autonomic functions.
