Yaşamın çeşitli dönemlerinde yaşanan travmatik olaylar kısa sürede olumsuz ruhsal değişimlere sebep olabilir. Bu değişimler ne kadar uzun sürdüklerine ve nelere yol açtıklarına bağlı olarak çeşitli patolojik tanıların belirtisi olabilir. Travma ile ilişkili bir bozukluk olan Geç Başlangıçlı Travma Sonrası Stres Bozukluğu (Geç Başlangıçlı TSSB) vakalarında ise stres belirtileri travmatik yaşantının üzerinden 6 ay veya daha fazla süre geçtikten sonra gözlenir. Mevcut belirtileri ve hastanın psikiyatrik öyküsünü irdelemek, sıkıntıların temelinde yatan başlıca sorunları ve travmaları tespit etmek ardından bu doğrultuda uygun tedavi yürütmek klinik tablo karmaşık olsa dahi hastalarda iyileşme sağlayabilir. Bu olguda çocukluk çağı cinsel taciz yaşantısının yetişkinlik döneminde tetiklenmesiyle ortaya çıkan ve farmakoterapiye yeterince olumlu cevap vermeyen Geç Başlangıçlı TSSB ile komorbid Majör Depresyonun EMDR terapisi ile tedavisi tartışılacaktır.

Traumatic events experienced in different periods of life may soon alter negative psychological changes. These negative shifts might be indications of pathology depending on their duration and consequences. In cases of Post Traumatic Stress Disorder with delayed expression (PTSD with delayed expression), which is a trauma-related disorder, stress related symptoms are observed after 6 months or later following traumatic event. Although the clinical picture be complicated, scrutinizing the patient’s existing symptoms and psychiatric history, detecting underlying problems and traumas, and then conducting a proper treatment accordingly may help provide recovery for its patients. In this case study, treatment of a PTSD with delayed expression comorbid Major Depressive Disorder (MDD) resistant to pharmacotherapy, which emerged after a childhood sexual trauma had become triggered in adulthood, will be discussed.
