Klinisyen ve araştırmacı olarak aileler ile karşılaştığımızda, obez ergenin ?semptomun sözcüsü? işlevi, bizi bu ikilemi incelemeye yöneltti: Pek çok ergen ailesinden uzaklaştığında kilo veriyordu, fakat eve geri döndüklerinde verdikleri kiloları alıyorlardı! Bununla birlikte aileler bilinçli olarak çocuklarının kilo vermesini isterken, bilinçdışı olarak ailesel işleyişlerini değiştirmeyi arzu etmezler. Ayrılma ve farklılaşma problemi olan bu ergenler kuruma yerleştirilerek, aile çevresi tarafından baskın gelenden, zayıflayarak ?ayrılmaya? zorlanırlar. Fakat aynı zamanda, aile grubuyla olan tüm farklılığı ve bireyselliği inkar ederek dengeyi sağlamak gerekir. Böylece, ailesel bir boşluğu, (nesiller arası ve ötesi) bir eksiği doldurmak üzere bireyin obez bedeni etrafında organize olan aile bağlarının yapısına ilişkin hipotezi ileri sürülebilir. Major obezite sebebiyle hastane yatışı olan ergen topluluğuna ait aile takip görüşmelerini içeren klinik bir olgu örneğinden hareketle; kimliğin inşa sürecindeki bazı eksikliklerin varlığını göstereceğiz. Narsisistik biçimde kurulan kimlikte, öteki, ayrışmış (farklılığın inkarı) bir halde ele alınmaz. Bireysellik, hayali ortak bir beden uğruna azalır.

Confronted as practitioner-researcher to the family, the function of ?portal symptom?of the obese teenager, has led us to wonder ourselves about this paradox: Too many teenagers löse weight when they are away from their family, but back home they take again the lost weight! However families consciously want that their child loses weight, but unconsciously do not wish to change their family functioning. Placed in institution, these teenagers whose problematic of separation and differentiation is proponderant of with their home environment are forced to ?separate?, while losing weight. But at the same time, they must maintain homeostasis, by denying any individuality and differentiation of with the family group. So, we put forward the hypothesis that the structuring of the family ties is organized around the obese body of the subject so as to fill a lack, a family vacuum (inter and trans-generational). Through an exemplary clinical case pulled by the follow-ups of family consultations of a population of teenagers hospitalized for major obesity; we shall show the existence of weaknesses in some of the processes of construction of the identity which establish under a narcissistic mode, where the other is not considered in a differentiated status (denial of the otherness). Individuality is reduced in aid of a common imaginary body.
