Amaç: Kohut’un kendilik psikolojisi kuramında kendilik gelişimi vurgulanmakta ve erken çocukluk ve ergenlik dönemlerinde aynalama, içselleştirme ve ayrışma süreçlerinde ortaya çıkan aksamaların kendilik patolojilerine yol açacağı belirtilmektedir. Benzer süreçler çocukluk ve ergenlik boyunca kimlik gelişimi yönünden de önem taşımaktadır. Bilindiği gibi kimlik gelişimi konusu en yoğun olarak ego psikolojisi kuramcılarından olan Erik Erikson tarafından işlenmiştir. Önceleri ‘ego kimliği’ tanımını kullanan Erikson bu konudaki çalışmalarının geç evrelerinde ego kimliğinin ‘kendilik kimliği’ olarak da ifade edilebileceğinden söz etmiştir. Kendi klinik deneyimlerimiz de ergenlerde görülen kimlik kargaşası tablolarının sıklıkla altta yatan kendilik patolojileri ile ilişkili olduğunu gösterdiğinden bu yazıda bir vaka örneğinden yola çıkılarak kimlik kargaşası ile kendilik patolojisinin ilişkisi incelenmiştir. Sonuç olarak, kimlik duygusunun kendilik organizasyonunda süreklilik duygusunu ifade eden bir kavram olduğu ve aynı zamanda kendiliğin nesnelerle ve sosyal çevreyle ilişkilerini de tanımladığı (sosyal kimlik, cinsel kimlik ve mesleki kimlik olarak) söylenebilir. Öyleyse kimlik, kendilik organizasyonunun ve işlevlerinin özel bir yanı olarak kabul edilebilir. Kimlik duygusunun gelişememesi, kendisinin nasıl biri olduğunu tanımlayamama gibi kimlik kavramına ilişkin sorunlar, bu yazıdaki olguda da görüldüğü gibi kendilik bütünleşmesinde aksamaların, yani bir kendilik patolojisinin göstergesi olabilecek klinik tablolar oluşturabilir.

Kohut described the disturbances in the mirroring, transmuting internalization and separation processes during childhood and the resulting self pathologies. Internalization and separation-individuation are also important in the process of identity formation during childhood and adolescence. Erik Erikson, who developed the theory on ego identity, in his later works stated that the term can also be used in the sense of ‘self identity’. He stated that there are both an ego and a self aspect of identity development. Our clinical experience with adolescents suffering from identity confusion showed us that the psychiatric symptoms of the clinical picture are psychodynamically related to underlying self pathologies. The purpose of this paper is to discuss the relation of identity development and identity confusion to self development and self pathology. The case history of a 20- [year]-old Muslim girl who ended up being a nun is presented. This case demonstrates how the disturbance of self development in the earlier [year]s may lead to a serious identity confusion in adolescence. Identity is a special aspect of self function in the integration process and sense of identity is a sense of wholeness and continuity of the self-images within the self organization. Problems related to identity, such as lacking a sense of identity or the inability to describe oneself, show that there is disturbance in the integration of the self, as is presented in this case. Thus, identity confusion is a sign of self pathology.
