Amaç: Transkraniyal manyetik stimulasyon (TMS) elektromanyetik prensipleri beyin korteksinde elektrik alan oluşturmak için uygulayan yeni bir yöntemdir. Elektrofizyolojide tanısal amaçlarla kullanımı oturmuş olup psikiyatrik hastalıkların tedavisindeki kullanımı üzerindeki çalışmalar ise yenidir. TMS ruhsal hastalıkların, özellikle depresyonun tedavisinde umut verici bir tedavi yöntemi olarak ortaya çıkmıştır. Bu çalışma TMS’un psikiyatrik uygulamalarındaki son gelişmelere dikkati çekmek için yapılmıştır.

Yöntem: Bu gözden geçirme yazısında TMS ile ilgili çalışmalar değerlendirilmiştir.

Bulgular: TMS yüksek frekanslarda kortikal aktiviteyi artırıyor ve düşük frekansta azaltıyor gözükmektedir. Elektrokonvülsif tedavinin aksine bu yöntemde etkili tedavi için bir konvülsiyon nöbeti gerekli değildir. Ayrıca anestezi gerektirmemesi ve geçici bellek bozukluklarına yol açmaması gibi önemli klinik üstünlükleri vardır. TMS’un depresyon belirtileri üzerinde yararlı etkileri olduğuna dair kuvvetli kanıtlar vardır, ancak bu etkiler geçicidir. TMS’un klinik uygulamaları ile ilgili eldeki veriler güvenli ve kabul edilebilir bir tedavi yöntemi olduğunu göstermektedir.

Sonuç: Uygun bir deneme döneminden sonra TMS depresyon için bir tedavi yöntemi olarak kabul edilebilir. Gelecekte şizofreni, posttravmatik stres bozukluğu, obsesif-kompulsif bozukluk ve mani gibi ruhsal bozuklukların tedavisinde de yeri olabilir.

Objective: Transcranial magnetic stimulation (TMS) is a new method that applies the principles of electromagnetism to deliver an electrical field to the cerebral cortex. Although this method is well established in diagnostic electrophysiology, its role in the treatment of psychiatric disorders is still under study. TMS has emerged as a promising therapeutic intervention for mental disorders, especially for depression. This study was conducted to draw attention to the latest developments in the psychiatric applications of TMS.

Method: In this retrospective review all relevant studies on TMS were evaluated.

Results: TMS seems to increase neural activation in the cerebral cortex at high frequency stimulation and decrease it at low frequency stimulation. In contrast to electroconvulsive therapy, TMS does not require the generation of a motor seizure for effective therapy. It also has important clinical advantages such as not requiring anesthesia and avoiding side effects like transient memory loss. There is strong evidence that TMS has beneficial effects on the symptoms of depression, but these effects are transient. Available data about the clinical applications of TMS suggests that it is a safe and acceptable technique for treatment.

Conclusions: After an appropriate trial period, TMS may become accepted as a treatment modality for depression. In the future, it may also have a place in the treatment of mental disorders such as schizophrenia, obsessive-compulsive disorder, posttraumatic stress disorder, and mania.
