Depressive disorder, contrary to belief, often begins at a much earlier age. Preschoolonset major depressive disorder (POMDD) develops as a result of cumulative interaction of early adverse environments and the ongoing acute and chronic stress factors with developmental processes. In POMDD, alterations in stress cortisol reactivity, in neural function, in neural system connectivity, and in brain volume were found. For POMDD, diagnostic criteria used in adolescents and in adults and modified appropriately for age is valid and reliable. The most sensitive symptom is irritability, and the most spesific symptom is anhedonia. Comorbidity rate is approximately 75%. For treatment of POMDD, psychotherapy as a first choice and, if the severe symptoms and functional impairment continue despite the psychotherapy, medication should be considered.
