Çocuk ve ergenlerde Tourette bozukluğu: Sosyodemografik, klinik özellikler ve eştanılar

Amaç: Bu çalışmada polikliniğimize başvuran çocuk ve ergenlerden Tourette Bozukluğu (TB) olanların sosyodemografik özelliklerini belirlemek, sosyodemografik özellikler ile motor ve vokal tiklerin şiddeti arasındaki ilişkiyi saptamak ve DSM-IV’e göre TB’ndaki eştanı sıklığını ve eştanıları belirlemek amaçlanmıştır.
Yöntem: Bu çalışmaya DSM-IV’e göre TB tanısı alan 28 çocuk ve ergen hasta (23 erkek, 5 kız) alındı. Hastaların sosyodemografik özellikleri kaydedildi ve Beck Çocuk Depresyon Ölçeği (ÇDÖ), Maudsley Obsesif Kompulsif Soru Listesi (MOKSL) ve Goetz Tik Derecelendirme Ölçeği uygulandı. Hastalar ile yapılan psikiyatrik görüşmeler sırasında hastalık belirtileri, eştanılar ayrıntılı olarak değerlendirildi.
Bulgular: Olguların başvurduklarında ortalama yaşları 124±32 aydı. Olguların 21’inde (%75) obsesif-kompulsif bozukluk (OKB), 4’ünde (%14.2) dikkat eksikliği hiperaktivite bozukluğu (DEHB), 4’ünde major depresyon eştanı olarak bulunmaktaydı. En yaygın motor tik göz kırpma, en yaygın vokal tik boğaz temizlemeydi. Olguların %32.1’inde koprolali vardı ve koprolali daha sık olarak ileri yaşlardaki çocuklarda görülmekteydi. Olguların prenatal, natal ve post-natal durumu ile TB arasında anlamlı nedensel bir risk faktörü saptanmadı.
Sonuç: TB’nun sosyodemografik özelliklerini incelediğimiz bu çalışmada, olguların büyük kısmında birlikte obsesif kompulsif belirtilerin, OKB ve/veya DEHB’nun bulunması bu hastalıkların etyolojisinde ortak noktalar bulunduğunu düşündürmektedir.
Tourette Disorder in Children and Adolescents: Sociodemographic, Clinical Features and Comorbidity
Objective: The aim of this study was to evaluate the sociodemographic and clinical features of children and adolescents with Tourette Disorder (TD) according to DSM-IV who are applied to our outpatient clinic and to determine clinical significancy of relationship between the sociodemographic features and the severity of vocal, motor tics, and also to determine frequency of comorbid conditions and diagnosis acording to DSM-IV.
Method: Twenty eight children and adolescents (23 boys, 5 girls) were included in this study. Sociodemographic features of cases were recorded and Beck Children Depression Inventory (CDI), Maudsley Obsessive Compulsive Questionnaire (MOCQ), Goetz Tics Evaluation Scale were administered to all children. Clinical features of TD and the comorbid diagnosis were assessed in detail thorugh psychiatric interview according to DSM-IV.
Results: The mean age of the cases at the time of referral was 124±32 [month]s. Twenty one of the cases (75%) had obsessive compulsive disorder, 4 of them (14,2%) had attention deficit hyperactivity disorder (ADHD), 4 of them (14.2%) had major depression as a comorbid disorder. The most common motor tic was eye blinking and vocal tic was throat cleaning. Coprolali was present in 32,1% of the cases was found to appear in older children. Besides, no significant causal relationship between prenatal, natal, and postnatal conditions of the cases and TD.
Conclusions: The results of the study suggest that there are several common pathologies in the etiology of TD, OCD, ADHD because of the high comorbidity present between these disorders.
