Depresif bozuklukta hipotalamo-pituiter-adrenal ve hipotalamo-pituiter-tiroid eksen bulguları

Amaç: Bu çalışmada, majör depresif bozuklukta (MDB) görülebilen nöroendokrin anormalliklerin nöbet ve iyileşme dönemlerinde ve kontrol grubu ile karşılaştırılması amaçlanmıştır.
Yöntem: Çalışmaya DSM-IV ve ICD-10’a göre MDB tanısı konmuş 20 hasta ve 20 sağlıklı denek rastgele örneklem yöntemiyle seçildi. Tüm deneklere Deksametazon Süpresyon Testi (DST) ve Tiroid Serbestleştirici Hormon (TRH) testleri uygulandı, bazal hormon düzeyleri ölçüldü.
Bulgular: Deksametazon öncesi 16:00 kortizol değeri dışında tüm kortizol yanıtları nöbet döneminde kontrol grubuna göre anlamlı olarak yüksek bulundu. Nöbet iyileşme dönemi karşılaştırmalarında ise deksametazon öncesi ve sonrası tüm kortizol değerleri anlamlı olarak nöbet döneminde yüksekti. TRH’ya Tiroid Uyarıcı Hormon (TSH) yanıtları, en yüksek (mak) TSH yanıtları, mak TSH düzeyinde değişim anlamlı olarak hasta grubunda düşüktü. DST pozitiflik oranı nöbet döneminde %70, iyileşme döneminde %5 olarak tespit edildi. TSH körelme oranı ise nöbet döneminde %50, iyileşme döneminde %35 olarak tespit edildi.
Sonuç: DST’nin nöbet döneminde pozitif olup iyileşme döneminde normale döndüğü, TSH yanıtındaki körelmenin hem nöbet hem iyileşme döneminde devam ettiği dikkati çekmiştir.
Hypothalamic-Pituitary-Adrenal and Hypothalamic-Pituitary-Thyroid Axis Findings in Depressive Disorder
Objective: To find out whether the neuroendocrine abnormalities, that may be seen in major depressive disorder (MDD), differ during depression and remission periods, compared with the controls.
Method: Twenty subjects, diagnosed as MDD according to DSM -IV, ICD-10, and 20 healthy controls were randomly selected and included in this study. Dexamethasone Suppression Test (DST) and Thyroid Releasing Hormone (TRH) test were performed and basal hormone levels were determined in all subjects.
Results: All cortisol responses were found to be significantly higher in the patient group during depression period compared to the control group except at 16:00 before dexamethasone. Comparison of depression and remission periods revealed that all cortisole levels before and after dexamethasone were significantly higher in depression period. Thyroid Stimulating Hormone (TSH) responses to TRH, maximal TSH responses, change in maximal TSH level were significantly lower in the patient group compared with the controls. The rate of DST positivity was 70% in depression period and 5% in remission period. Blunted TSH response rate was 50% in depression period and 35% in remission period.
Conclusion: We found that while DST positivity in depression returned to normal during remission, blunted TSH response was consistent both during depression and remission.
