Menopozda hormon yerine koyma sağaltımının anksiyete ve depresyon belirtilerine etkisi

Amaç: Menopoz döneminde azalan hormonların yerine konması biçiminde yapılan sağaltım (hormon yerine koyma sağaltımı, HYS) önemli bir sağaltım yöntemidir. HYS’nin menapoz döneminde gelişebilen psikiyatrik belirtileri nasıl etkilediğine ilişkin olarak araştırmacılar arasında tam bir görüş birliği yoktur. Bu çalışmada, menopoz döneminde HYS amaçlı kullanılan östrojen ve tibolonun, anksiyete ve depresyon belirtilerine etkilerinin araştırılması amaçlanmıştır.
Yöntem: Bu çalışmaya Kadın Hastalıkları ve Doğum Polikliniği’ne başvuran, doğal yoldan menopoza girmiş kadınlar alınmıştır. Denekler (s=70) rastgele iki gruba ayrılarak bir gruba 17b-östradiol, diğer gruba ise tibolon tedavisi verilmiştir. Tüm hastalar sağaltım öncesinde ve 3 aylık sağaltım sonrasında Hamilton Anksiyete Derecelendirme Ölçeği (HADÖ) ve Hamilton Depresyon Derecelendirme Ölçeği (HDDÖ) ile değerlendirilmiştir.
Bulgular: Çalışmaya alınan 70 hastanın 46’sı çalışmayı tamamlamıştır. Östrojen kullanan hasta grubu (s=23) ile tibolon kullanan hasta grubunun (s=23) yaş ortalamaları, eğitim düzeyleri ve meslek dağılımları farklı bulunmamıştır. Tibolon grubunda menopoz süresi daha uzundu. Hastaların HADÖ ve HDDÖ puanlarında sağaltım öncesine göre istatistiksel olarak anlamlı bir düşüş bulunmuş ve iki ilaç arasında bu açıdan fark olmadığı saptanmıştır.
Sonuç: HYS menopoz dönemindeki kadınlarda anksiyete ve depresyon belirtilerinde düzelme sağlamaktadır. Bu etki açısından 17b-östradiol ve tibolon arasında fark yoktur.Mesin Ü Tıp Fak.
The Effects of Hormone Replacement Therapy in Menopause on Symptoms of Anxiety and Depression
Objective: Replacement of deficient hormones (hormone replacement therapy, HRT) is the main treatment modality in menopause. There is no concensus among researchers as to how HRT influences psychiatric symptoms that might develop during menopausal period. In this study we aimed to explore the effects of HRT on symptoms of anxiety and depression comparatively by using either estrojen or tibolone.
Method: Women with natural menopause who applied to the outpatient Clinic of Obstetrics and Gynecology Department were included in the study. Subjects (n=70) were randomized into two groups with one group to receive 17b-estradiol and other group tibolon. All subjects were evaluated at baseline and after 3- [month]s of HRT with Hamilton Anxiety Rating Scale (HARS) and Hamilton Depression Rating Scale (HDRS).
Results: Forty six of 70 patients completed the study. The 17b-estradiol (n=23) and the tibolone (n=23) groups were similar with respect to age, education and occupational status. The time from onset of menopause was longer in the tibolon group. HARS and HDRS scores were significantly decreased after 3 [month]s of HRT in both the 17b-estradiol and the tibolone groups. The two drug regimens were not significantly different in this respect.
Conclusion: HRT with 17b-estradiol and tibolone improves symptoms of anxiety and depression in menopausal women.
