Rett Sendromlu olgularda beyin SPECT bulguları

Amaç: Rett sendromlu (RS) 8 olguda ve aynı yaş grubunda 8 kontrol olgusunda Tc99m-HMPAO SPECT ile bölgesel serebral kan akımım ölçmek ve bulguları karşılaştırmak. Yöntem: Rett sendromlu 8 olguda ve aynı yaş grubunda 8 kontrol olgusunda Tc99m-HMPAO SPECT uygulandı. Rett sendromlu tüm olgulara aynı zamanda bilgisayarlı tomografi de yapıldı (ve bulgular sadece 2 olguda hafif serebral atrofi ile uyumlu idi). Bölgesel serebral kan akımı değerlendirmesi hem vizuel hem de semikantitatif olarak değerlendirildi ve transaksiyel kesitlere toplam 27 adet ilgi alanı çizilerek bu alanlardaki ortalama sayımlar elde edildi ve beyin bölgesi/ tüm beyin oranları hesaplandı. Bulgular: RS olgularında anterior frontal-prefrontal, parietal ve temporal bölgelerde kontrol grubu ile karşılaştırıldığında istatistiksel olarak anlamlı perfüzyon farkı izlenmekte idi (p<0.05). Tartışma: Bu çalışma Tc99m-HMPAO SPECT'in yüksek kortikal merkezleri ilgilendiren işlevsel anomalileri ortaya koymada değerli olduğunu ve diğer anatomik görüntüleme ve laboratuvar bulgularının normal olduğu göz önüne alındığında RS'de, işlevsel görüntüleme yöntemlerinin bölgesel beyin değişikliklerini saptamada daha duyarlı olduğunu göstermektedir.
Brain SPECT findings in cases with Rett Syndrom
Objective: To investigate regional cerebral blood flow using technetium 99 m hexamethylpropyleneamineoxime single photon emission computed tomography (SPECT) in 8 children presenting with Rett syndrome and compare with 8 age comparable subjects with normal brain SPECT findings. Method: 8 children presenting with symptoms of Rett syndrome were included and all patients were also examined by computed tomography (And findings harmonious with mild cerebral atrofi only 2 cases). Abnormalities in regional blood flow (rCBF) on SPECT were determined both by visual and semiquantitative evaluation by means of drawing 27 irregular regions of interest and obtaining cortical to whole hemispheric ratios. Results: The rCBF values obtained from the RS group were significantly lower than the control group (p< 0.05) in bilateral frontal, parietal and temporal regions. Conclusion: We conclude that Tc99m HMPAO brain SPECT is valuable in the exploration of the functional abnormalities involving the higher cortical centers in RS and functional imaging is superior for the detection of the pathology in the absence of abnormalities by laboratory and other diagnostic modalities in RS.
