Amaç: Bu çalışmanın amacı: özellikle hastanelerde çalışan psikiyatristlere, deliryumunun yaygınlığı, konsültasyon nedenleri, klinik bulguları ve tedavi yaklaşımları hakkında bilgi vermektir ve yoğun konsültasyon hizmeti veren bir merkezin verilerini sunmaktır. Yöntem: Bu yazıda İstanbul Tıp Fakültesi Psikiyatri A.B.D., Konsültasyon Liyezon Psikiyatrisi (KLP) Bilim Dalı’ nda yataklı servislere verilen konsültasyon hizmeti çerçevesinde, 1995-1998 yılları arasında değerlendirilen 270 deliryum olgusunun verileri sunulmuştur. Bulgular: Hastalar en sık cerrahi servislerinde (%33.3), en az da dermatoloji servisinde (%0.7) görülmüştür. Hastaların birincil tanıları arasında en sık çeşitli sistem kanserleri (%22.5), ikinci sırada da akut genel travma (%17) belirlenmiştir. Konsültasyonlar en sık ajitasyon ve eksitasyon (%44.3) nedeniyle istenirken, psikiyatrik muayenede en sık bulgu "zaman yöneliminde" bozulmadır (%76.8). Etiyolojik neden olarak metabolik bozuklukların ön planda olduğu saptanmıştır. En sık elektrolit denge bozukluğu (%28.3) deliryuma neden olmuştur. Olguların %39.9'unda Mixt tip, %38.4'ünde Hiperaktif-Hiperalert tip ve %8.7'sinde Hipoaktif-Hipoalert tip deliryum belirlenmiştir. Tartışma: Deliryumun tanınması ve gerek fiziksel hastalığın gerekse psikiyatrik belirtilerin tedavisi hastalardaki morbidite ve mortalitenin azaltılabilmesi açısından önemlidir. Ancak elde edilen veriler halen deliryumun az tanındığını, depresyon gibi hastalıklarla karıştırıldığını ve hastanelerimizdeki bakım şartlarının halen eksikliklerle dolu olduğunu göstermektedir.

Objective: The aim of this study was to inform psychiatrists who are working in hospital settings, about the insidance, causes of consultation, clinical symptoms, and treatment approaches of delirium and to present the data of a centre providing a high rate of consultation service. Method: This paper reports; 270 delirium cases evaluated on the basis of in-hospital consultation in between 1995-1998, by the Istanbul University Medical Faculty Psychiatry Department, Consultation and Liaison Psychiatry Division. Results: Most of the consultation demand was from the surgical departments (33.3%) and the least from the dermatology department (0.7 %). The most common primary underlying illnesses were cancers (22.5%) and generalized body trauma (17 %). Agitation and excitation were the main causes for a consultation request; and "time disorientation" was the main psychiatric symptom (76.8 %) evaluated by the consultant psychiatrists. Metabolic disorders were dominant as etiological causes of delirium: electrolyte imbalances being the majör one (28.3 %). 39.9% mixt type, 38.4 hyperactive-hyperalert type and 8.7 %> hypoactive-hypoalert type delirium cases were encountered. Conclusion: Recognition of delirium and the treatment of both physical disease and psychiatric symptoms at the same time is very important in terms of decreasing mortality and morbidity of the patient. But this study shows; delirium is underestimated, confused with other disaease such as depression and the level of hospital çare is stili unsatisfactory.
