Somatizasyonu olan hastalarda anksiyete bozuklukları ve depresyon

Amaç: Bu çalışmada, organik bozukluğu olmaksızın psikiyatri dışı kliniklerde çeşitli bedensel yakınmalarla tedavi arayışında olan hastaların 1. eksen tanılarını ve komorbiditelerini, sosyodemografık özelliklerini, cinsiyetin yakınmalara etkisini, somatik yakınma sürelerini, anksiyete, depresyon ve hipokondriyakkaygı düzeylerini, somatik tedavi sıklığını ve somatik yakınma tiplerini araştırmayı amaçladık. Yöntem: Haziran 1998-Kasım 1998 tarihleri arasında iç hastalıkları, kardiyoloji, fizik tedavi ve nöroloji bölümlerinden organik bir bozukluğu bulunamadığı gerekçesiyle psikiyatri polikliniğine gönderilen 117 hasta çalışmaya alındı. Bunlar; 15-70 (33.81±10.93) yaş aralığında 79 kadın ve 15-68 (31.78±12.25) yaş aralığında 38 erkek hastaydı. Hastalara SCID-1 (The Structured Clinical Interview for DSM111-R), Beck Depresyon Ölçeği (BDÖ), Sürekli Anksiyete Ölçeği (SAÖ) uygulandı. Bulgular: Hastaların 41 'i (%35) çoğul tanı, 76'sı (%65) tekil tam aldı (41 somatoform bozukluk, 19 duygudurum bozukluğu, 13 anksiyete bozukluğu, 3 uyum bozukluğu). Tekil tanılarla birlikte çoğul tanılar ayrı ayrı sayıldığında 117 hastaya toplam 187 tam kondu (76 somatoform bozukluk, 66 duygudurum bozukluğu, 42 anksiyete bozukluğu, 3 uyum bozukluğu). Hastaların BDÖ puanı ortalaması (22.16 ± 13.02) ve SAÖ puanı ortalaması (52.28 ± 9.74) yüksek düzeyde bulundu. Tartışma: Somatizasyon; ruhsal bozukluklarda özellikle de depresyon ve anksiyete bozukluklarında yüksek oranda bulunur. Gastrointestinal yakınmalar daha çok depresyon, kardiyopulmoner yakınmalar ise anksiyete ile ilişkili bulunmuştur. Bedensel yakınmaların süresi ile ilişkili olarak depresyonun düzeyi ve sıklığı artmaktadır.
Anxiety disorders and depressive disorders with somatization patients
Objective: The aim of this study was to evaluate the relationship between the following parameters in patients with non-organic somatic complaints who have applied to different outpatient clinics: axis-l diagnoses and comorbidities, sociodemographic charecteristics, changes in somatic complaint according to gender, duration of somatic complaints, levels of anxiety and depression, hypochondriac anxiety levels, frequency of somatic therapies and type of somatic complaints. Method: All patients who were admitted to other outpatient clinics and sent for diagnosis to outpatient clinic of psychiatry for unexplained somatic symptoms, during June 1998 and November 1998 were enrolled into the study. There were 117 patients in the study group, 79 females at 15-70 ages (33.81H0.93), and 38 males at 15-68 ages (31.78±12.25). Their diagnoses were made through SC1D I (The Structured Clinical lnterview for DSM III-R) by a psychiatrist. In addition, Beck Depression Inventory (BDI) and State-Trait Anxiety Inventory (STAI-trait) were given to patients. Results: 41 patients had multiple diagnoses and 76 patients had single diagnoses. Distributions of the 76 patients having single diagnoses were 41 patients with somatoform disorders, 19 patients with mood disorders, 13 patients with anxiety disorders and 3 patients with adjustment disorders. Distributions of the 187 single and multiple diagnoses were 76 somatoform disorders, 66 mood disorders, 42 anxiety disorders and 3 adjustment disorders. BDI and STAI-trait scores were found to be significantly high. Conclusion: Somatization was found to be frequent in psychiatric disorders particularly in depression, and anxiety disorders. Gastrointestinal symptoms were correlated with depression and cardiopulmonary symptoms were correlated with anxiety disorders. Frequency and degree of depression were increased with related to duration of somatic complaints.
