Major depresif bozuklukta kan pıhtılaşma sistemi

Amaç: Bu çalışmanın amacı major depresif bozuklukta (MDB) kan pıhtılaşma sisteminin etkilenip etkilenmediğini, etkilenme varsa bunun hangi mekanizmada ve düzeyde olduğunu göstermektir. Yöntem: Çalışmaya Diagnostic and Statistical Mental Disorders-IV (DSM-IV) ölçütleri ve Hamilton Depresyon Ölçeği'ne (HDÖ) göre MDB tanısı konan, halen kan pıhtılaşma sistemine etkileyebilecek hastalık ve ilaç kullanma öyküsü olmayan, antidepresanlarla tedaviye başlanmamış 60 hasta alınmıştır. Kontrol grubuna ise MDB'u olmayan, kan pıhtılaşma sistemini etkileyebilecek hastalık ve ilaç kullanma öyküsü olmayan, hastalarla benzer sosyodemokrafik özelliklere sahip 60 birey alınmıştır. Hasta ve kontrol gruplarındaki kanama zamanı (KZ), pıhtılaşma zamanı (PZ), aktive parsiyel tromboplastin zamanı (APTZ), protrombin zamanı (PTZ) sonuçları t testi, tekrarlı ölçümlerin sonuçları ise varyans analizi ile karşılaştırılmıştır. Bulgular: Hasta grubunun başlangıç KZ, PZ ve APTZ değerlerinin kontrol grubunun değerlerine göre anlamlı düzeyde bozulduğu; PTZ değerleri arasında ise anlamlı fark bulunmadığı görülmüştür. KZ değerlerinin hastaların % 35'inde fizyolojik sınırların dışına çıktığı saptanmıştır. PZ, APTZ ve PTZ fizyolojik sınırların içinde bulunmuştur. Tartışma: Bu bulgular ışığında MDB trombosit fonksiyonları ve intrensek pıhtılaşma mekanizmasının bozulduğu, bu bozukluğun trombosit fonksiyon bozukluğu ile ilgili olabileceği ve fizyolojik sınırların dışına çıkmadığı söylenebilir. Ekstrensek pıhtılaşma mekanizmasının bozulduğuna ilişkin yeterli bulgu elde edilmemiştir.
Blood coagulation system in major depressive disorder
Objective: This study was performed in order to find out whether coagulation system was disturbed in major depresive disorder (MDD) in patients or not if so, to determine the underlying mechanism and level of disturbance. Method: 60 patients who were diagnosed to have MDD and did not recieve any antidepressant medication were included in the study. Patients did not have any history of disease and medication which might impair the coagulation cascade, a control group was constitued with 60 persons who were not diagnosed to have MDD, and who did not receive any medication which could effect coagulation cascade and who did not have any disease that can effect coagulation system and who had the same sociodemographic characteristics both the patients group. As a results of resarches like the bleeding time (BT), the coagulation time (CT), the activated parcial thromboplastine time (APTT) and the prothrombin time (PT) between patients and control group was compaired for student-t test and the results of the repetetively measures was compaired with variance analys. Results: The results of the repetetively measures were impaired significantly in patients group. Results of BT experiment were over the physiological ranges in 35 % of patients. However results of CT, APTT, and PT test was between the physiologic ranges. There were not any statistically significant results at PT asseys between control and patient group. Conclusion: With the help of this data, it can be concluted that thrombocyte functions and intrinsic coagulation cascade are impaired in patients with MDD, impairment of intrinsic coagulation mechanizm due to the disturbance of thrombocyte functions and it is not out of the physiological ranges. Enough data was not obtained about impairment of extrinsic coagulation mechanism
