Genel sağlık anketi: Üç ayrı puanlama yöntemine göre geçerlik ve test - tekrar test güvenirlik

Amaç: Klasik GSA, C-GSA ve Likert puanlama yön temleri kullanıldığında GSA-12'nin özellikle kronik hastalar açısından tarama başarısını saptamak ve test- tekrar test güvenirliğini göstermektir. Yöntem: Değişik şikayetlerle birinci basamak tedavi birimine başvuran 110 hastaya GSA-12 ve SCID uygulanmış ve bunlardan 60'ı güvenirlik çalışması için 4-5 gün sonra tekrar çağrılmışlardır. Testler değişik puanlama yöntemleriyle değerlendirilmiş ve sonuçlar ROC analizi ile karşılaştınlmıştır. Bulgular: Hem Pearson korelasyon tekniği hem de "iki eş arasındaki farkın önemlilik testi" ile saptanan güvenirlik her üç puanlama metodu için de yeterli bulunmuştur. Üç puanlama yöntemi ile de benzer tarama özellikleri saptanmıştır; hiçbirinin tek başına diğerinden üstün olmadığı, GSA ve C-GSA yöntemleri birlikte kullanıldığında performansın arttığı ortaya çıkmıştır. Tartışma: GSA-12"nin iki aşamalı çalışmalarda ağır olmayan psikiyatrik hastalıkları taramada başarılı bir araç olduğu sonucuna varılmış, iki puanlama yönteminin beraber kullanılması önerisi getirilmiştir.
General Health Questionnaire: Validity for three scoring methods and test-retest reliability
Objective: The primary aim of the study was to verify the test-retest reliability and to detect the screening performance of GHQ-12 especially for chronic patients, using different scoring; Method: conventional GHQ sco¬ring, C-GHQ scoring and Likert scoring. Method: GHQ- 12 and SCID were given to 110 patients attending to a primary health care center with different complaints, 69 of whom were invited to fill GHQ-12 again after four or five days for tests-retest reliability. Scoring was done by using the three different methods and the results were compared by ROC analysis. Results: The test-retest relia¬bility, detected by both Pearson's Correlation and Paired t-test, was satisfactory for each scoring method. The sco¬ring methods showed similar screening properties. None of them had any superiority to another alone. The perfor¬mance was improved by combining the conventional GHQ and C-GHQ case criteria. Conclusion: GHQ-12 had a satisfactory performance for screening of minor psychiatric disorders in a two stage design. Using two scoring methods together was recommended to get the best results.
