Obsesyon, kompulsiyon ve psikotik belirtileri olan ve obsesyondan aşın değer verilmiş düşüncelere ve sanrılara uzanan bir spektrumda yer alan hastalar uzun süredir literatürde yer almaktadır. Psikotik belirtileri olan, "içgörüsü olmayan” obsesif kompulsif bozukluk (OKB) sıklıkla tedaviye yeterli yanıt vermemektedir. Son zamanlarda yapılan epidemiyolojik ve klinik çalışmalarda, şizofreni ile obsesif-kompulsif belirtilerin birlikte bulunabileceği ve kötü prognozun belirleyicisi olduğu belirtilmektedir. Her iki grubun klinik özellikleri ve tedavi yaklaşımları bu örneklerin yer aldığı olgular verilerek tartışılmıştır.

Patients with, obsessions, compulsions and psychotic symptoms have long been described in the literature. Patients appeared to move on a continuum from obsessions to over valued ideas to delusions. OCD with psychotic features, "OCD without insgiht" are often treatment refractory and present significant therapeutic challenges. Recent epidemiological and clinical studies indicate coexitance of obsessive compulsive symptoms with schizophrenia and predicts a worse clinical course and poorer treatment outcome. The literature on the clinical characteristics, treatment studies of OCD with psychotic features and schizophrenia with obsessive compulsive symtoms are reviewed with related case examples.
