Amaç: Fibromiyalji sendromunda bedensel ve psişik belirtiler görülür ve bu psişik etmenlerin, hastalığın kliniğinde önemli rol oynadığı düşünülmektedir. Bu çalışma; aleksitimi ile öfkenin, hastalığın kliniğinde oynadığı rolü araştırarak, fibromiyaljinin daha iyi anlaşılması amacıyla planlanmıştır.

Yöntem: Çalışma, Fiziksel Tıp ve Rehabilitasyon polikliniğine başvuran fibromiyalji sendromu tanısı konan 101 kadın hasta ile romatoid artrit tanısı konan 30 kadın hasta ve 59 sağlıklı kontrolle yürütülmüştür. Bireyler Görsel Analog Ölçeği, 20-Maddelik Toronto Aleksitimi Ölçeği, Spielberger Sürekli Öfke ve Öfke İfade Tarzı Ölçeği, Beck Anksiyete Ölçeği, Beck Depresyon Ölçeği, Fibromiyalji Etkilenme Anketi ve sosyodemografik özellikleri sorgulayan bir görüşme formu kullanılarak değerlendirilmiştir.

Bulgular: Gruplar yaş, eğitim yılı, medeni durum ve ekonomik durum açısından birbirine benzer yapıda idi. Fibromiyalji sendromlu grupta anksiyete ve içe döndürülmüş öfke puanları, diğer gruplardan anlamlı ölçüde yüksek bulundu. Depresyon ve aleksitimi puanları, fibromyalji grubunda sağlıklı kontrol grubuna oranla anlamlı ölçüde yüksek bulundu.

Sonuç: Bu bulgular fibromiyalji hastalarının romatoid artritli hastalara oranla daha fazla anksiyete ve öfke-içe yaşadıklarını göstermektedir. Hasta grupları sağlıklı kontrol grubundan daha fazla aleksitimik olmasına rağmen kendi aralarında anlamlı fark bulunmadı. Bu durum bir öfke ifade tarzı olan bastırılmış ve içe döndürülmüş öfkenin, yüksek anksiyeteyle birlikte hastalığın bir parçası olduğunu düşündürmektedir.

Objective: Fibromyalgia syndrome is characterizted by both somatic and psychic symptoms and it is suggested that psychic factors contribute to the clinical presentation of this syndrome. This study was planned to have a better understanding of fibromyalgia through elaborating the role of alexithymia and anger in the pathogenesis of this illness.

Method: The study was carried out in the Physical Therapy and Rehabilitation outpatient clinic with 101 women with fibromyalgia syndrome, 30 women with rheumatoid arthritis and 59 healthy women with no current or past medical history. The subjects were evaluated by the Visual Analog Scale, Toronto Alexithymia Scale-20 items, Spielberger State-Trait Anger Inventory, Beck Depression Scale, Fibromyalgia Impact Questionnaire and a sociodemographic data form.

Results: All these groups were similar to each other in means of age, [year]s of education, marital and economical status. In the fibromyalgia syndrome group, the scores of anxiety and anger-in were calculated significantly higher than other groups. The depression and alexithymia scores were found higher than healthy group.

Discussion: These findings suggest that fibromyalgia patients suffer more anxiety and anger toward oneself, which is anger-in, than rheumatoid arthritis patients. Though the patient groups were more alexithymic than the healthy group, alexitimia scores of the two patient groups were not different. This situation suggest that anger-in, which is suppressed and unexpressed anger style is a part of the fibromyalgia syndrome together as well as high anxiety.
