Majör Depresyona Eşlik Eden Anksiyete Belirtilerinin Venlafaksin XR’ın Tedavi Sonuçlarına Etkisi

Amaç: Bu çalışmanın amacı depresyon ve anksiyete tedavisinde noradrenerjik sistemin
önemine yapılan sık vurgulardan yola çıkarak, bir serotonin ve noradrenalin gerialım inhibitörü
(SNGİ) olan venlafaksin XR tedavisinin depresyona ve eşlik eden anksiyete belirtileri
üzerine olan etkisinin değerlendirilmesidir.
Yöntem: Bu amaçla DSM-IV majör depresif bozuk tanı ölçütlerini karşılayan ve çalışma
protokolüne uygun olan toplam 43 hasta alındı. Çalışma ilacı olan venlafaksin XR 1. vizitte
75 mg/gün olarak başlandı, 2. vizitte ise 150 mg/ gün’e çıkıldı ve sonraki 8 hafta boyunca
doz sabit tutuldu. Çalışma süresi olan on bir hafta boyunca toplam altı kez değerlendirilen
hastalara her değerlendirmede Hamilton Depresyon Derecelendirme Ölçeği (HAM-D) ve
Hamilton Anksiyete Derecelendirme Ölçeği (HAM-A) uygulandı.
Bulgular: Çalışmaya dahil olan hastaların 2. vizitten itibaren HAM-A ve HAM-D skorlarında
anlamlı düşüş olduğu ve her iki ölçek skorunda düşüşün birbirleriyle korele olduğu, anksiyetesi
olan grupta (tarama vizitinde HAM-A≥22) epizot sayılarının ve başlangıç HAM-D
skorlarının daha yüksek olduğu saptanmıştır. Anksiyöz grubun depresyon skor ortalamaları
nın her vizitte daha yüksek olduğu bulunmuştur. Çalışma sonunda anksiyetesi olmayan
grupta, remisyona (HAM-D≤10) girme ve anksiyete yanıt oranlarının daha yüksek olduğu
saptanmıştır. En sık ağız kuruluğu, mide bulantısı,sersemlik ve baş ağrısı yan etkileri saptanmı
ştır. Yan etki nedeniyle tedaviyi bırakan olmamıştır.
Tartışma ve Sonuç: Özetle anksiyete varlığı, venlafaksin XR ile tedavide yanıt ve remisyonu
azaltmakta, ortaya çıkan yanıt ve remisyonu ise geciktirmektedir. MDB’a eşlik eden
anksiyetenin bu etkisi tedavi seçimi açısından önemlidir. Hangi antidepresan tedavi veya
hangi sistem (serotonin, noradrenalin veya her ikisi) üzerine etkinin depresyona eşlik eden
anksiyete üzerine etkililiği, karşılaştırmalı çalışmalarla test edilmelidir.
The Effects of Comorbid Anxiety Symptoms to the Treatment Outcome of Major Depressive Disorder With Venlafaxine XR
Objective: The aim of this study is to evaluate the efficacy of venlafaxine in major depressive
disorder (MDD) with coexisting anxiety symptoms.
Method: Study subjects consisted of 43 patients, who met DSM-IV MDD diagnostic criterion.
Duration of the study was 11 weeks. Venlafaxine 75mg/day was started at visit 1, increased
to 150 mg/day at visit 2 and the dosage remained unchanged until the end of the
study. Hamilton Depression Rating Scale (HDRS) and Hamilton Anxiety Rating Scale
(HARS) were applied to each subject 6 times during the study.
Findings: At the end of visit 2 significant decreases were observed in both HDRS and HARS scores. Decreases in scale scores were correlated. Initial HAMD scores and number
of previous depressive episodes were higher in anxious group (HARS≥22 at study admission).
Mean HARS scores were significantly higher in anxious group. Depression remission
rates (HDRS≤10) and response rates of anxiety to treatment (HARS score reduction
>50%) at the end of the study were both significantly higher in nonanxious group. Most prominent
side effects were dry mouth, nausea, drowsiness and headache. No drop outs were
recorded due to side effects.
Discussion and Conclusion: Presence of anxiety reduces the rates and delays the onset
of remission and response to venlafaxine treatment. Studies comparing the efficacy of antidepressants
in major depressive disorder with coexisting anxiety symptoms are needed.
