Dikkat Eksikliği ve Yıkıcı Davranış Bozukluklarında Doğum Öncesi, Doğum ve Doğum Sonrası Yaşanan Sorunların ve Anne Sütü Alış Sürelerinin Psikopatoloji Gelişimine Katkıları

Amaç: Dikkat Eksikliği Hiperaktivite bozukluğu (DEHB) ve DEHB'ye karşıt olma-karşı gelme bozukluğu (KOKGB) eşlik eden erkek çocukların; cinsiyet, yaş, sosyoekonomik düzey, zeka düzeyi bakımından eşleştirilmiş kontrol grubu ile karşılaştırılarak; doğum öncesi, doğum sırasında ve sonrasında yaşanan sorunların ve anne sütü alış süresinin psikopatoloji gelişimine olan katkılarının araştırılması amaçlanmaktadır. Yöntem: Çalışmaya, Dokuz Eylül Üniversitesi Tıp Fakültesi çocuk psikiyatrisi polikliniğine başvuran 6-11 yaş arasında DEHB tanısı konan 21, DEHB+KOKGB tanısı konan 26, sağlıklı gönüllülerden oluşan kontrol grubuna 27 erkek çocuk alınmıştır. Doğum öncesi, doğum ve doğum sonrasına ilişkin bilgilerin sorgulandığı sosyodemografik veri formu doldurulmuştur. Tanıların desteklenmesi amaçlı; Dikkat Eksikliği ve Yıkıcı Davranış Bozuklukları için DSM-IV? e dayalı Tarama ve Değerlendirme Ölçeği, anneler tarafından, Conners Öğretmen Derecelendirme Ölçeği kısa formu öğretmenler tarafından doldurmuştur. Bulgular: Gebelikte annelerde psikososyal stresörle karşılaşma öyküsü, DEHB+KOKGB grubunda kontrol grubuna göre anlamlı düzeyde daha fazla saptanmıştır. Anne sütü alış süresi; DEHB grubunda, kontrol grubuna göre anlamlı düzeyde kısa bulunmuştur. Tartışma-Sonuç: Annelerin gebeliğinde yaşadığı stresin, hipotalamik-pituiter-adrenal (HPA) ekseni işlevlerine olumsuz etki yapmış olabileceği ve bu yolla DEHB+KOKGB gelişimine katkıda bulunmuş olabileceği izlenimi vermektedir. Anne sütünün DEHB gelişimi üzerine koruyucu etkisi olabileceği düşünülmektedir.
The Effects of Prenatal, Perinatal and Postnatal Problems and Breast Feeding Duration on the Development of Psychopathology in Attention Deficit and Distruptive Behavior Disorders
Objective: The aim of this study was to investigate the effects of prenatal, perinatal and postnatal problems and the duration of breast feeding on the development of psychopathology in attention deficit and disruptive behavior disorders (DBD). Methods: 21 boys with a diagnosis of ADHD and 26 boys with ADHD and comorbid oppositional defiant disorder (ODD), who were seeking medical treatment for the first time at the child psychiatry outpatient clinic of Dokuz Eylül University Faculty of Medicine and a control group consisted of 27 healthy volunteer boys were included in the study. Sociodemographic data, prenatal, natal and post natal histories and the duration of breast feeding of children were obtained by a sociodemographic data form. The parents filled DSM-IV based behavior disorders screening form and rating scale for attention deficit and disruptive behavior disorders, and the teachers filled Conners Teacher Rating Scale. Results: It was found out that the mothers' encounter with psychosocial stressors during pregnancy were significantly higher in ADHD and comorbid ODD group compared to the control group. The duration of breast feeding was significantly shorter in children with ADHD than in controls. Conclusion: It seems that the stress experienced by mothers during pregnancy might have had a negative effect on the functions of hypothalamic-pituitary-adrenal axis and by this way contributed to the development of ADHD and ODD. Breast feeding might prevent the development of ADHD.
