Çocuklar ve ergenler değişen ağırlıkta ve çeşitli görünümlerde klinik sorunlar nedeniyle acil servise gelmektedirler. Acil hekimi değerlendirmesinde psikiyatrik acil durumların ağırlık, ciddiyet ve risklerini saptamalıdır. Öneriler acil tedavi planlarını içermeli ve hastanın tedaviye uyumunu kolaylaştırmalıdır. Acil müdahalede amaç hastanın hastalık öncesi işlev düzeyine dönmesini sağlamaktır. Multidisipliner bir tedavi ekibinin varlığı bu süreci kolaylaştırabilir. Pediyatrik acil servis başvurularında görülen psikiyatrik durumlar arasında sıklıkla duygudurum bozuklukları, anksiyete bozuklukları, davranışsal sorunlar, entoksikasyon ve madde kullanımıyla ilişkili sorunlar yer almaktadır. Acil değerlendirmesinin çocuk psikiyatrisi hekimi, acil hekimi, sosyal hizmet uzmanı ve hemşire tarafından oluşturulan bir ekip tarafından yapılması en uygun olanıdır. Klinik değerlendirmede akut psikiyatrik bozukluk, akut ruh sağlığı sorunları ve akut tıbbi komplikasyonları hızlı ve sistematik şekilde taramak kritik önemdedir. Değerlendirmenin amaçları tanısal değerlendirmenin yanı sıra çocuğun ya da ergenin fiziksel ve ruhsal bütünlüğünü bozabilecek risklerin ve başlatıcı, sürdürücü ve koruyucu faktörlerin belirlenmesini, sosyal destek sisteminin tanımlanmasını kapsamalıdır. Değerlendirme sonrasında hızlı bir ilk müdahale planı ve tedavi/takip planı yapılmalıdır. Bu yazının amacı, çocuk ve ergen psikiyatrisi acili alanında sıkça karşılaşılabilecek durumların tanımlanması ve çocuk ve ergen grubuyla acil koşullarında çalışan uzmanların ortak yaklaşım geliştirmesine yol gösterilmesidir.

Managing Emergencies in Child and Adolescent Psychiatry Children and adolescents present to the emergency room (ER) for a diverse array of clinical problems at various degrees. ER physician should be able determine the significance, severity and risks of the psychiatric emergency conditions in her evaluation. The recommendations should include acute treatment plans and facilitate patient’s adherence to further treatment. The purpose in acute treatment is to enable the patient to return to the premorbid functioning level. The presence of a multidisciplinary treatment team may facilitate this process. In pediatric ER applications, the most common psychiatric cases are mood disorders, anxiety disorders, behavioral problems, suicide attempts, intoxication and problems related to substance use. The emergency team should ideally include a child psychiatrist, an ER physician, a social worker and a nurse. In the clinical evaluation, rapid and systematic screening of an acute psychiatric disorder and acute medical complications has crucial importance. The purposes of the evaluation should include identification of the risk factors associated with possible damages to children or adolescent’s physical and mental health, precipitating, sustaining and protective factors, and social support systems. After the evaluation, acute treatment and follow-up plans should be made. The purpose of the current study is to define the common child and adolescent psychiatric conditions in the ER and to guide the physicians who have to manage the psychiatric conditions in the pediatric ERs.
