Bedensel cinsiyetiyle örtüşmeyen bir cinsiyet kimliği olan transgender bireyler, psikiyatri kliniklerine başvurduklarında, gerçek kimlikleri ile bireysel ve toplumsal yaşamlarında var olma mücadelesi vermektedirler. Topluma egemen olan ikili cinsiyet düzeni, bireyleri katı bir şekilde sınırları çizilmiş erkek ve kadın cinsiyet kategorilerine sığmak zorunda bırakmaktadır. Bu da kişilerin bedensel özellikleriyle ve kendilerinden beklenen cinsiyet ifadesi ile yoğun bir çatışma içerisine düşürmektedir. Bu durum psikiyatride cinsiyetinden hoşnutsuzluk olarak tarif edilmektedir. Toplumun olumsuz tutumu birey tarafından da içselleştirilerek olumsuz benlik imajı gelişmesine, kimi zaman ciddi ruhsal bozuklukların belirmesine neden olabilmektedir. Kişi gerçek kendini dışa vurduğunda ailede, okulda, işte ve sokakta damgalanma, dışlanma ve ayrımcılığa maruz bırakılmaktadır. Psikiyatrinin görevi kişiye yansız ve yargısız bir ilişki içinde kendini tanıma imkanı sağlamak ve bütünlüklü bir kimlikle arzuladığı şekilde toplum içinde yaşamını sürdürmesine destek olmaktır. Geçiş süreciyle kişi cinsiyet kimliğine uygun sosyal görünüme ve bedensel özelliklere sahip olabilmektedir. Bu süreç yasal bir çerçevede sürdürülen hormon tedavileri ve cerrahi işlemler gibi tıbbi müdahaleleri de içermektedir. Bireyin kendine özgü talep ve beklentileri, tıbbi koşullar, yasal süreç, bireysel ve çevresel imkan ve sınırlılıklar değerlendirilerek, bireye en uygun şekilde uzun vadeli bir geçiş planının yapılması gereklidir. Ruh sağlığı uzmanı birden çok disiplin tarafından sürdürülen bu değerlendirme ve izlem sürecinin planlanmasından başlayarak her basamağında önemli rol oynamaktadır. Psikiyatrın bu süreçte bireyin yanı sıra ailesi ve yakınlarına da destek sağlaması önemlidir. Psikiyatr tarafından bu sürecin etik ilkeler ve bilimsel araştırmalar doğrultusunda güncellenen uygulama kılavuzları doğrultusunda yönetilmesi, bireyin bedensel, ruhsal ve sosyal yönden iyilik halinin sağlanması ve sürdürülmesine hizmet etmektedir

Gender identity issues from childhood to adulthood: appropriate evaluation and follow-up Transgender individuals, whose gender identity is incongruent with their sex, seek psychiatric care as a part of their struggle to express their real identity in both personal and social life. Widely held idea of gender binaries requires individuals to fit into strictly defined gender categories as man or woman, which, in these individuals, creates conflicts related to their sex characteristics and gender role. This condition is called gender dysphoria in psychiatry. Negative attitude of the society may be internalized by the individual, which contributes to the development of a negative self image and mental health problems. The individual experiences stigmatization and discrimination in the family, in school, at work, and in the street, if the identity is expressed. The role of psychiatry is to create a gender neutral and non-judgemental context where the person can explore his/her gender identity freely, and to provide support for the individual to express it the way he/she prefers. Social and physical characteristics congruent with the gender identity are achieved through the transition process. Transition process also includes medical interventions like hormone treatment and surgical procedures, which should be executed in line with the legal regulations. The expectations and the demands, medical conditions, legal status, opportunities and obstacles of the person should be evaluated in order to construct the most appropriate long-term individualized transition plan. Mental health professionals have important role in the planning and execution of every step in this evaluation and follow-up, which frequently involves multiple disciplines. Support for the family should always be included into the plan. The management of the transition process by the psychiatrist with the guidance of the ethical principles, according to the standards of care provided by professional organizations, serves best to the achievement and preservation of the physical, mental and social well-being of the individuals
