Obsesif kompulsif bozukluğu (OKB) olan hastaların önemli bir kısmı ilaç tedavisi ve bilişsel davranışçı terapilerin (BDT) birlikte kullanımında bile tatmin edici bir tedavi yanıtı vermekte yetersiz kalmaktadır. OKB’nin fenomenolojik özellikler açısından yüksek oranda çeşitlenebilir doğası nedeniyle, bu çeşitlilik ve tedavi yanıtı arasındaki ilişki araştırmacıların artan bir şekilde ilgisini uyandırmaktadır. Bununla birlikte ilaç çalışmaları belirti grupları hakkında istifleme belirtileri dışında başarılı bir tedaviyi açıkça yordayan tutarlı sonuçlar ortaya koyamamıştır. Diğer taraftan BDT, OKB’de tüm belirti grupları için tedavinin önemli bir parçası olmuştur. BDT için çalışma sonuçları bulaşma/temizleme ve zarar görme/kontrol grubu belirtilerin alıştırma ve tepki engelleme tekniklerine daha iyi yanıt verdiğini ortaya koyarken, bilişsel terapi dini/cinsel grubu belirtiler için daha etkili bulunmuştur. Özetle belirti çeşitliliğine bakmaksızın antidepresan tedavi ve BDT’nin birlikte kullanımı OKB tedavisinin temel dayanağı olarak durmaktadır. Bununla birlikte fenomenoloji ve tedavi direnci arasındaki ilişkiyi anlamak için daha geniş kapsamlı bir yaklaşıma ihtiyaç bulunmaktadır. OKB hastaları bazı tanı ötesi özelliklerin obsesif kompulsif belirtiler üzerine olan etkisi nedeniyle hastalığın çekirdek belirtilerini farklı bağlamda tecrübe edebilirler. Bu yazıda, eldeki tedavi seçeneklerinin belirti boyutlarına etkisi ve tanı ötesi özelliklerin bu etkiye nasıl aracılık edebilecekleri tartışılmıştır.

Even after combined treatment strategies including pharmacotherapy and cognitive behavioral therapy (CBT), a significant percentage of patients with obsessive-compulsive disorder (OCD) fail to experience a satisfactory response. Because of highly heterogeneous nature of in terms of phenomenological properties, the relationship between phenomenological heterogeneity and response to treatment has increasingly aroused interest of researches. However, pharmacotherapy studies have not been able to reveal consistent results about symptom profiles as the clear predictors of successful treatment response except for hoarding. On the other hand, CBT interventions have become an important part of the management of all symptom dimensions in OCD. Data suggest that patients with contamination/cleaning or doubts about harm/checking symptoms appear to respond well to exposure and ritual prevention whereas cognitive therapy has been found more effective for religious-sexual obsessions and related compulsions. In sum, combined usage of antidepressants and CBT remains the mainstay of treatment of OCD, regardless of OCD symptom subtype. However, to better understand the relationship between phenomenology and treatment resistance a comprehensive approach is required. The OCD sufferers can experience core diagnostic elements in different context because of the impact of some transdiagnostic phenomenological properties on obsessions and compulsions. In this review, the impact of the available treatment options on symptom dimensions and probable modulating and intervening mechanisms of transdiagnostic properties are discussed.
