Türkiye'de psikiyatristlerin özerklik ve paternaliteye ilişkin ikilemler hakkında tutumları

Amaç: Bu çalıuşmada Türkiye’deki psikiyatristlerin hastaların istemlerine karşın tutumlarının paternalizm ve otonomi(özerklik) arasındaki çatışmaları içeren örnek olaylar bağlamında saptanması ve bunu etkileyebilecek muhtemel faktörlerin belirlenmesi amaçlanmıştır.
Yöntem: 67 soruluk ve yarı-yapılandırılmış bir anket formunun 13 sorusu ile hekimlere sosyodemografik verileri,teorik-felsefi, psikiyatrik yönelimleri;5 ayrı klinik duruma ilişkin tavırları sorulmuştur.Bu sorularla hastanın üreme hakkı,tedaviyi reddetme hakkı gibi konularda psikiyatristlerin hasta istemine karşı tutumları araştırılmıştır.
Bulgular: Tanımlanan örnek olaylara karşı hekim tutumlarının,hekimlerin sosyodemografik verileri ve teorik-felsefi,psikiyatrik yönelimleriyle anlamlı bir ilişkisi olmadığı anlaşılmıştır.Hastanın aksi yönde istemine rağmen bir tıbbi uygulama(zorla yatırma) konusunda en yüksek oranda hekim tutumu psikotik olmayan süisidal,depresyon(%75) ve borderline kişilik(%71) olgularında gerçekleşmiştir.Hamile kalabilmek için ilacının kesilmesini isteyen remisyonda duygudurum hastasına ve vazektomi olmak isteyen şizofreni hastasının istemlerine karşı tutum sırasıyla %59 ve %50 oranlarında gerçekleşmiştir.Manipülatif bir intihar girişimi ile başvuran ve yatarak tedavi görmek istemeyen histrionik kişilik vakasında hasta istemine karşı tutum en az oranda gözlenmiştir(%18).
Tartışma: Çalışmanın bulguları Türkiye’deki psikiyatristlerin burada sunulan örnek olaylar üzerinde,özerklik konusundaki tutumlarında hastanın karar verme kapasitesinden daha çok tıbbi endişeleri göz önünde bulundurduklarını düşündürmektedir.
Attitudes of Turkish psychiatrists about the dilemmas related with autonomy and paternality
Objective: This study aimed to investigate the attitudes of psychiatrists about the conflict of autonomy versus paternalism in physician-patient ralationship in the context of case vignettes and the possible related factors.
Method: A semi-structured questionnaire was used that consist of 67 questions,5 of which were about sociodemographical variables and 3 of them were vignettes about theoretical-philosophical,and psychiatric views of psychiatrists.5 questions were vignettes about patients autonomy and paternalism conflict.By using these guestions,attitudes of psychiatrists towards the patients’ rights to reproduce,and to refuse the treatment were explored.
Results: As a response to all of the vignettes,attitudes of psychiatrists were found not to be related with their theoretical-philodophical and psychiatric wievs and sociodemographical variables.The highest response rates against patients’ demands were observed in the vignettes containing suicidal depressive(%75) and borderline(%71) patients.Rates of the attitudes against the demands of the mood disorder patient(who is in remission and wants to give up the prophylactic treatment to become pregnant) and schizophrenic patient(who refuses the vasectomy operation) were found %59 and %50 respectively.Histrionic patient case was the least one considering the response rates against patient’s autonomy(%18).
Discussion: Results of the study suggested that the attitudes of Turkish psychiatrists in the context of case vignettes presented in the study were related with their medical priorities rather than the choice and decision making capacity of the patients.
