Üç Tolosa-Hunt sendromu olgusu: Klinik seyir ve MRG takipleri

Tolosa-Hunt sendromu, ağrılı oftalmopleji ile prezente olan, kavernöz sinüs/superior orbital fissür’ün idiopatik granulomatöz inflamatuvar hastalığıdır. Unilateral orbital ağrıya eşlik eden 3, 4, 6. kraniyal sinirler ile 5. kraniyal sinirin oftalmik ve maksiller dallarından biri veya daha fazlasının tutulumu mevcuttur. Kraniyal MRG’de T1 sekanslarda izointens, T2 sekanslarda izo-hipointens kavernöz sinüse ait genişleme ve IV kontrast verilmesini takiben belirgin kontrast tutulumu görülmektedir.
Unilateral orbital ağrıyı takiben çift görme yakınması eklenen, nörolojik muayenelerinde tek veya multipl kraniyal nöropati saptanan, kraniyal MRG tetkiklerinde kavernöz sinüsde kontrast tutan lezyon gösterilebilen, steroid tedavisi ile 72 saat içinde ağrısı kaybolan, üç hafta içinde oftalmoplejisi düzelen üç olguda Tolosa-Hunt sendromu düşündük ve sık görülen bir tablo olmaması nedeniyle sunmayı uygun gördük.
Tolosa-Hunt sendromu nadir görülen bir tablo olup, unilateral ağrılı oftalmopleji ile başvuran ve steroid tedavisine kısa bir zaman içinde dramatik şekilde yanıt verdiği gözlenen hastalarda ilk planda olası tanı olarak düşünülmesi gereken hastalıklardan biri olmalıdır.
Tolosa-Hunt Syndrome: Clinical and Radiological Follow-up
Tolosa-Hunt syndrome is described as painful ophthalmoplegia due to idiopathic granulomatous involvement of the cavernous sinus/superior orbital fissure. Unilateral orbital pain associated with paralysis of one or more of the third, fourth and sixth cranial nerves are the clinical findings. In addition to the involvement of the ocular motor nerves, ophtalmic and maxillary branches of the fifth cranial nerve may be affected. Cranial MRI reveals enlargement of the affected cavernous sinus by an abnormal tissue isointense with gray matter on T1-weighted images and iso-hypointense on T2-weighted images. This abnormal tissue markedly increases in signal intensity after contrast injection.
We followed up three cases in our clinic. All of the patients described unilateral orbital pain preceding diplopia. On neurologic examination there were one or multiple cranial neuropathies. Cranial MRI demonstrated enlargement of cavernous sinus with positive contrast enhancement. After steroid therapy, orbital pain disappeared within 72 hours and ophthalmoplegia improved within three weeks. The diagnosis was Tolosa-Hunt syndrome based on these findings in all patients.
Although Tolosa-Hunt syndrome is an uncommon condition, it should be considered as a possible diagnosis on the patients who are admitted with painful ophthalmoplegia and respond promptly to steroid therapy in a short period.
