Başağrıları migrenöz semptomlar taşıyan hastalarda ilk görüşmede tanı konulmasında karşılaşılan güçlükler

Amaç: Başağrılı hastalara ilk görüşmede Uluslarası Başağrısı Derneği (International Headache Society; IHS) kriterlerine göre tanı koymak mümkün olmayabilir. Hastaların takiplerinde tanılarının ne oranda netleştirilebileceği bu çalışmada araştırılmıştır.
Metod: Baş ağrıları migrene ait semptomlar içeren ve ilk görüşmede klinisyen tarafından kuvvetle migren tanısı düşünülen, ancak IHS’ye göre "migren" tanı kriterlerini karşılamayan ve IHS 13 ve 1.7 numaraları ile kodlanan 22 hasta izlenmiştir.
Bulgular: Bu hastalar içinden 6 hasta kontrol muayenelerine gelmedikleri için kesin tanıları konulamamıştır. En az 3 ay süre ile takip edilen 16 hastadan 4’üne kontrol muayenelerinde bildirdikleri atak özellikleri 7’sine başağrısı günlüğü aracılığı, 5’ine ise ampirik tedavi uygulanarak tanıları hakkında daha kesin kanı elde edilmiştir.
Sonuç: İlk görüşmede tanı koyma güçlüğü ile karşılaşılan hastaların belli sürelerle izlenmesi gerekmektedir; bu sebeple hastalara kontrol muayenelerinin önemi vurgulanmalıdır.
Diagnostic Difficulties in the First Interview with Patients Whose Headaches Have Migrainous Symptoms
Objective: It could not be possible to diagnose headache patients according to IHS criteria in the first interview. In this study, rate of ending up with the definite diagnosis was investigated in the follow-up of these patients.
Methods: The twenty two patients taken in this study suffered from headaches which include migraine symptoms and the clinician strongly considered migraine diagnosis. However, the headache symptoms of these patients didn’t meet the migraine criteria of IHS and they were coded 13 and 1.7 according to IHS criteria.
Results: Because of these patients did not attend the follow-up examinations, a definite diagnosis could not be established. Among the 16 patients who were followed for at least three [month]s, more precise conclusions about diagnosis were obtained in 4 of them by attack peculiarities that they reported during the following interviews, in 7 by headache diary and in 5 by empirical therapy.
Discussion: Patients whose headache type cannot be identified during the first interview should be followed within certain periods and patients should be encouraged for follow-up examinations.
