Travma yaşantısı olan kadınlarda grup psikoterapisinin etkinliği

Amaç: Bu çalışmada kronik seyirli, Travma Sonrası Stres Bozukluğu (TSSB) tanısı almış kadınlarla etnik farklılıklara ve cinsiyete duyarlı yaklaşımla yapılan bir psikoterapi modelinin etkinliğinin değerlendirilmesi amaçlanmıştır.
Yöntem: Çalışmaya Güney Doğu Anadolu Bölgesinde yaşayan TSSB tanısı almış veya ağır TSSB semptomları olan 33 kadın olgu alınmıştır. Olgulara Travma Sonrası Stres Tanı Ölçeği Olayın etkilerini değerlendirme ölçeği, Stresle Başaçıkma Tarzları Ölçeği ve travma yaşantılarının özelliklerini kapsayan sosyodemografik form uygulanmıştır. Çalışma süresi ayda iki tam gün (12 saat), toplam 60 saati tamamlayacak şekilde 5 aydır.
Bulgular: Olguların yaş ortalaması 31.18 (± 6.7) idi, %91’i TSSB tanısı almaktaydı. %76’sının travmatik yaşantısı birden fazlaydı ve %91’i şikayetlerinin üç aydan uzun zamandır sürdüğünü belirtmiştir. Çalışma sonunda olguların 3/4’ünün TSSB tanısı ortadan kalkmıştır. Uygulanan ölçeklerin semptom şiddeti, işlev kaybı, TSSB tanıları ön- son test sonuçları arasında semptomların iyileşmesi yönünde istatistiksel olarak anlamlı fark bulunmaktadır (p<0.05). Bu fark iyileşmenin devamı şeklinde yapılan takip çalışmalarında da sürmüştür (p<0.05). Uygulanan model terapinin sonuçları açısından etkili görülmektedir.
Sonuç: Çoğul ve uzun süreli travmatik etkenlere maruz kalan ve TSSB tanısı alan kadınlarla yapılan, cinsiyete duyarlı bir grup psikoterapi uygulamasının etkin olduğu gözlendi. Bir kadın merkezinin işbirliğiyle yapılan bu çalışma, TSSB tanısı alanlarda, farklı terapi modellerinin bütünleştirildiği, heterojen, etnik yapı ve cinsiyete duyarlı yaklaşımıyla, ülkemizde örnek bir model oluşturmaktadır. Kullanılan tedavi modelinin TSSB tanısı alan kadınlarda, iyileşme ve iyileşmenin devamı lehinde sonuçlarının olduğunun görülmesi aynı konuda yapılacak çalışmalarda etkili bir model olacağını göstermektedir. Sonuç olarak TSSB tanısı almayan %9’luk grup tedavi sonunda %91’e yükselmiştir.
Effectiveness of Group Psychotherapy in Women with Traumatic Life Events
Objective: In this study the effectiveness of a psychotherapy model sensitive to ethnic differences and gender in women diagnosed with Post Traumatic Stress Disorder (PTSD) with chronicity was aimed to be assessed.
Method: 33 women diagnosed with PTSD or having severe PTSD symptoms and currently living in the South East Anatolian region were included in this study. The cases were given Post Traumatic Stress Scale (PDS), Impact of Event Scale Revised (IES-R), Strategies for Coping With Stress Scale (SCWSS) and a sociodemographic questionnaire concerning the details of traumatic life events. The study was completed in a total of 60 hours corresponding to 5 [month]s, which was carried out in 2 full days (12 hours) per [month].
Results: Mean age of the cases was (31.18 ± 6.7) and 91% of the cases were diagnosed as PTSD. 76% had more than one traumatic life event and 91% had symptoms lasting for more than three [month]s. At the end of the study PTSD symptoms have been resolved in 3/4 of the cases. There was a statistically significant difference (p<0.05) favoring resolution of symptoms between pre and post test results in all the scales used as far as symptom severity, function loss, and PTSD diagnosis were concerned. This difference was further seen in the follow up studies (p<0.05). The treatment model applied in this study seems effective as seen in the results of treatment.
Conclusion: Group psychotherapy sensitive to gender applied to women who had been subject to multiple and sustained trauma and with PTSD diagnosis proved to be effective. This study, carried out with the collaboration of a women’s center, sets an example in our country with its heterogeneous approach and sensitivity to ethnicity and gender combining a variety of treatment models in the treatment of women with PTSD. Results obtained in the treatment of women with PTSD using this model, such as resolution of symptoms in short term and follow-up, show that it might be an effective model for future studies concerned with the same subject. As a result, at the end of the study the ratio of women not meeting criteria for PTSD diagnosis arise from 9% to 91%.
