Selektif Mutizm (SM), konuşabilme becerisinin olmasına rağmen konuşmanın beklendiği bir takım ortam ve durumlarda konuş(a)mama olarak tanımlanmaktadır. Tedavisinde ilaç tedavisi ve psikoterapötik girişimlerin birlikte kullanılması önerilmektedir. SM’nin geçmişten günümüze anksiyete temelli bir bozukluk olduğunun düşünülmesi ve son yayımlanan DSM-5’te anksiyete bozuklukları sınıfında yer alması nedeniyle özellikle davranışsal ve bilişsel-davranışsal terapiler son yıllarda sıklıkla kullanılan terapi seçenekleridir. Anksiyolitik etkilerinden dolayı selektif serotonin geri alım inhibitörleri (SSGI) SM tedavisinde en sık kullanılan psikofarmakolojik ajanlardır. Bu yazıda okul öncesi dönemde kliniğimize başvuran selektif mutizm tanılı iki kız olgunun tanı ve tedavi süreci ile birlikte fl uoksetinin semptomlar üzerindeki etkisinin tartışılması amaçlanmıştır

Selective Mutism (SM) is identifi ed as consistent failure to speak in specifi c settings and situations in which there is an expectation for speaking though speech capacity is not impaired. Combined interventions that would integrate medication and psychotherapeutic approaches are recommended for treatment. Since it has long been believed that SM was an anxiety-based disorder and currently being categorized within anxiety disorders regarding the latest publication of DSM-5 diagnostic classifi cation, behavioral and cognitive-behavioral therapies in specifi c have been among the most frequently used treatment options for the disorder, lately. Selective serotonin reuptake inhibitors (SSRIs) are the most commonly used psychopharmacologic agents in the treatment of SM due to their anxiolytic effects. This article aims to discuss the effect of fl uoxetine on the symptoms along with the presentation of diagnostic and treatment processes of two girls with selective mutism who had applied to our unit while they were preschoolers.
