Şizofreni Hastalarında Tekrarlayan Yatış Sıklığı ve Öngörücüleri

Amaç: Bu çalışmada, yatarak tedavi gören şizofreni tanılı hastalarda, taburculuk sonrası 6 ay içerisinde yeniden yatış sıklığının ve bunu etkileyen sosyodemografik ve klinik değişkenlerin incelenmesi amaçlandı. Yöntem: Bu amaçla, 01 Nisan 2011- 30 Eylül 2011 tarihleri arasında, hastanemizde yatarak tedavi görmüş ve taburcu edilmiş, şizofreni tanılı 292 hasta çalışmaya dahil edildi. Bulgular: Taburculuk sonrasındaki 6 aylık süre içinde 292 hastanın 50'si (%17.1) yeniden hastaneye yatmıştır. İlk kez hastane yatışı olan hastaların hiç biri bu süre içerisinde yeniden hastaneye yatmamıştır. Tekrarlayan yatışı olan hastaların ortalama yeniden yatış zamanı 85.6±52.1 gündür. Tekrarlayan yatış riski ailede psikoz öyküsü olanlarda 2.3 kat daha fazla görülürken hastalığın başlangıç yaşının küçülmesiyle arttığı, yatış sayısının artışıyla birlikte arttığı, yatış ve taburculuk PANSS puanlarının artışıyla birlikte arttığı görüldü. Sonuç: Yatarak tedavi gören şizofren hastaların izleminde, ailede psikoz öyküsü, hastalığın başlangıç yaşı, önceki yatış sayısı ve hastalığın şiddeti yeniden yatışı etkileyen etkenlerdir.
Frequency and Predictors of Recurrent Hospitalizations of SchizophrenicPatients
Objectives: In this study, it was aimed to investigate the
frequency of re-hospitalizations within 6 months after
discharge and sociodemographic and clinical variables
affecting it in schizophrenic patients who were treated
at the hospital. Method: For this purpose, 292 schizophrenic
patients who were hospitalized, treated at our
hospital and discharged from the hospital between April
10th 2011 and September 30th 2011 were included in
the study. Sociodemographic and clinical data (positive
and negative syndrome scale: PANSS) were obtained
from medical records. Results: Fifty (17.1%) of 292
patients were re-hospitalized within 6 months after discharge.
None of the patients who had been hospitalized
for the first time were re-hospitalized during this period.
Mean duration of re-hospitalization is 85.6±52.1 days.
While re-hospitalization risk was 2.3 fold greater among
the patients who had the family history of psychosis, it
was seen to decrease as the age of onset got smaller,
increase as the number of hospitalizations and PANSS
scores on hospitalization and discharge increased.
Conclusion: Family history of psychosis, age of onset of
the disease, number of previous hospitalizations and disease
severity are the factors which affect re-hospitalization
in follow up of schizophrenic patients who are hospitalized.
