Fibromiyalji ve Romatoid Artritte Çocukluk Çağı Örselenmeleri, Anksiyete ve Depresyon Düzeyleri

Giriş: Kronik ağrı, anksiyete, depresyon ve çocukluk örselenmelerinin birbirleriyle yakın
ilişkisi bilinmektedir. Bu çalışmanın amacı kronik ağrı ile giden hastalıklar olan fibromiyalji
ve romatoid artrit tanısı alan hastalarda çocukluk örselenmelerinin, anksiyete ve
depresyon düzeylerinin araştırılması ve sağlıklı kontrollerle karşılaştırılmasıdır.
Yöntem: Çalışmaya fibromiyalji tanılı 30 hasta, romatoid artrit tanılı 30 hasta ve hastalarla
cinsiyet, yaş ve eğitim açısından eşdeğer 30 sağlıklı kontrol alındı (toplam 90 katılımcı).
Tüm katılımcılara sosyodemografik veri formu, Çocukluk Örselenme Yaşantıları Ölçeği
(ÇÖYÖ), Hastane Anksiyete ve Depresyon Ölçeği (HAD) uygulandı. Ayrıca hastalardan
Sayısal Ağrı Skalası (SAS) doldurması istendi.
Bulgular: Fibromiyalji grubunun ÇÖYÖ duygusal kötüye kullanım ve HAD depresyon
puanları kontrol grubundan; HAD anksiyete puanı ise hem kontrol grubundan, hem de
romatoid artrit grubundan anlamlı olarak daha yüksekti. Romatoid artrit grubunun ise
ÇÖYÖ duygusal kötüye kullanım ve HAD depresyon puanları kontrol grubundan anlamlı
olarak daha yüksekti. Fibromiyalji grubunun SAS puanı romatoid artrit grubundan daha
yüksekti. HAD anksiyete ve HAD depresyon puanları eşik üstü olanlarda ÇÖYÖ cinsel
kötüye kullanım düzeyi daha yüksekti.
Sonuç: Hem fibromiyalji, hem de romatoid artritte çocukluk örselenme yaşantıları
ve depresyon düzeyi yüksektir. Ağrılı hastalığı olanlarda çocukluk örselenmelerinin,
anksiyete ve depresyonun araştırılması tedavi başarısı açısından önemlidir.
Childhood Traumatic Experiences, Anxiety, and Depression Levels in Fibromyalgia and Rheumatoid Arthritis
Introduction: The close relationship between chronic pain, anxiety, depression, and
childhood traumatic experiences is well known. The aim of this study is to investigate
childhood traumatic experiences, anxiety, and depression levels in patients with
fibromyalgia and rheumatoid arthritis, which are diseases that cause chronic pain.
Method: A total of 30 patients with fibromyalgia, 30 patients with rheumatoid arthritis, and
30 healthy controls, matched with patients with respect to gender, age, and education,
were included in the study (90 participants in total). All participants were given a form
for sociodemographic characteristics, the Childhood Trauma Questionnaire (CTQ), and
Hospital Anxiety and Depression Scale (HAD). Patients were also asked to complete a
numeric pain scale (NPS).
Results: Patients with fibromyalgia reported significantly higher scores for CTQ
emotional abuse and HAD depression compared with healthy controls. Patients with
fibromyalgia reported significantly higher scores for HAD anxiety than both healthy
controls and patients with rheumatoid arthritis. Patients with rheumatoid arthritis
reported significantly higher scores for CTQ emotional abuse and HAD depression
compared with healthy controls. Pain scores of patients with fibromyalgia were higher
than in patients with rheumatoid arthritis. Participants who had scores over the threshold
on HAD anxiety and depression had significantly higher scores on CTQ sexual abuse.
Conclusion: Both patients with fibromyalgia and patients with rheumatoid arthritis have
high levels of childhood traumatic experiences and depression. Patients with painrelated
disorders should be examined for childhood traumatic experiences, anxiety, and
depression for better treatment outcomes.
