Amaç: Çalışmamızda hayatla bağdaşmayan ciddi fetal anomali saptanan gebelerde veya normal yaşamı önemli bir şekilde etkileyecek fetal anomali varlığında gebeliğini sonlandırmaya karar veren ve gebeliği sonlandırılan kadınlarda, mizaç ve karakter özelliklerinin, Travma Sonrası Stres Bozukluğu (TSSB) gelişmesi üzerinde etkili olup olmadığının araştırılması amaçlandı. Yöntem: Çalışmaya İstanbul Bakırköy Kadın Doğum ve Çocuk Hastalıkları Hastanesi Kadın Doğum Kliniğine Mayıs-Ağustos 2010 tarihleri arasında başvuran, fetal anomali nedeniyle gebeliğinin sonlandırılmasına karar veren 80 kadın hasta alındı. Bilgilendirilmiş Onay Formu ile çalışmaya katılmayı kabul eden katılımcılar sırası ile Sosyodemografik ve Klinik Görüşme Formu, Edinburg Doğum Sonrası Depresyon Ölçeği (EDSDÖ), Başa Çıkma Tutumlarını Değerlendirme Ölçeği (BÇTDÖ), Mizaç ve Karakter Envanteri (MKE) ile değerlendirildi. Gebelik sonlandırılması uygulandıktan altı ay sonra kontrol muayenelerinde katılımcılara Klinisyen Tarafından Uygulanan Travma Sonrası Stres Bozukluğu Ölçeği (TSSB-Ö) uygulandı. Bulgular: Gebelik sonlandırılması uygulanan kadınların %62,5 da travma sonrası stres bozukluğu, %65 olguda ise doğum sonrası depresyon bulguları tespit edildi. Katılımcılardan TSSB tanısı alanlarda yenilik arayışı, zarardan kaçınma ve ödül bağımlılığı mizaç özellikleri anlamlı derecede yüksek saptandı. TSSB tanısı alan ve doğum sonrası depresyon riski olanların, eğitim düzeyi ve ekonomik durumları daha düşük idi. Yine bu grupta, sosyal ve mesleki işlevsellikteki düşüş anlamlı bulundu. TSSB tanısı almayan grupta duygusal odaklı başa çıkma biçiminin yüksek oluşu dikkat çekici idi. Sonuç: Henüz gebelik sonlandırılması kararı aşamasındayken belirli mizaç ve karakter alt boyutları ön planda olduğunda, bu gruba TSSB ve depresyon gelişme ihtimali düşünülerek psikiyatrik destek verilmesinin uygun olacağı düşünülmektedir.

Objective: The present study aimed to investigate whether the temperament and character profiles have an effect on the development of post-traumatic stress disorder (PTSD) in pregnant women who were detected to have a baby with severe fetal anomalies incompatible with life or those who decided to terminate the pregnancy for the presence of fetal anomaly that may significantly affect normal life. Method: The present study included eighty women who attended to the Gynecology Clinic at Istanbul Bakirkoy Gynecology and Children's Diseases Hospital between May and August 2010 and decided to undergo termination due to fetal anomaly. The participants were informed about the study and written informed consent was obtained from all subjects who were assessed by sociodemographic and clinical interview form, the Edinburgh Postnatal Depression Scale, Coping Strategies Inventory (COPE), and Temperament and Character Inventory, respectively. Six months after the termination, the Clinician-Administered PTSD Scale (CAPS) was administered to the participants at the follow-up examination. Results: 62.5% of women who had terminated the pregnancy had PTSD and 65% of participants had symptoms of postpartum depression. The participants, who were diagnosed with PTSD, had significantly higher scores on novelty seeking, harm avoidance and reward dependence temperament sub-scales. The women who were diagnosed with PTSD and those who had risk of postpartum depression were found to have lower education level and economic status. Besides, in this group, social and occupational functioning was significantly lower. Emotion-focused coping style was prominently higher in women who were not diagnosed with PTSD. Conclusion: In cases of certain temperament and character profiles, psychiatric support during the decision process of termination is suggested to be important for reducing the risk of PTSD and depression development.
