Bipolar Bozukluk ve Mizaç İlişkisi

Güncel araştırmaların sonuçları, duygudurum ilişkili özgün mizaç tiplerinin (siklotimik ve hipertimik), bipolar bozukluğun yaygın ve öncül eşikaltı (trait) görüngüleri olduğunu göstermektedir. Toplumda %20'ye varan oranda gözlenen duygudurum ilişkili mizaç tipleri arasında, depresif, siklotimik ve anksiyöz mizaç, kadınlarda daha sık iken, hipertimik ve irritabl mizaç erkeklerde baskındır. Moleküler genetik çalışmaları, bu mizaç tiplerinin varlığında, duygudurum epizoduna genetik yatkınlık ile, merkezi serotoninerjik (depresif, siklotimik, irritabl ve anksiyöz mizaç) ve dopaminerjik (hipertimik mizaç) aktivite arasında güçlü bir etkileşim olduğunu ileri sürmektedir. Hastalık öncesi mizaç tiplerinin, minör ve major duygudurum epizodunun polarite ve belirti oluşumu yönünden klinik olarak değerlendirilmesinde, önemli bir rolü vardır. Bunlar, aynı zamanda hastalığın uzun süreli gidişini ve özkıyım, yeme bozukluğu, madde kullanım ve dürtü kontrol bozukluğu ve diğer kendine zarar verici davranışları içeren sonuçlarını anlamlı ölçüde etkileyebilirler.
Relation of Bipolar Disorder and Temperament
Recent studies show that affective temperaments (depressive, cyclothymic, hyperthymic, irritable and anxious) are the subsyndromal (trait-related) manifestations and commonly the antecedents of bipolar disorders. Up to 20% of the population has some kind of marked affective temperaments, depressive, cyclothymic and anxious temperament is more frequent in women, whereas hyperthymic and irritable temperaments predominate among men. Molecular genetic studies show a strong involvement of the central serotonergic (depressive, cyclothymic, irritable and anxious temperaments) and dopaminergic (hyperthymic temperament) regulation, suggesting that the genetic potential of major mood episodes lies in these temperaments. The type of premorbid affective temperaments have an important role in the clinical evolution of minor and major mood episodes including the direction of the polarity and the symptom formation of acute mood episodes. They can also significantly affect the long-term course and outcome including suicidality and other forms of self-destructive behaviours such as eating disorders, substance use and impulse control disorders.
