Psikotik bulgulu depresyon depresyon alt tiplerinden en şiddetli olanıdır. Sık yineleme, daha fazla yetiyitimi, kötü klinik gidiş ve yüksek mortalite ile ilişkilidir. Genetik faktörler ise psikotik bulgulu depresyonun duygudurum-psikotik spektrumdaki bozukluklarla çakıştığını göstermektedir. Yanlış tanı konma oranları yüksektir. Birçok tedavi klavuzu tedavide ilk sıra olarak antidepresan ve antipsikotik tedavi kombinasyonunu veya EKT'yi önermektedir. EKT özellikle eştanılı durumlarda ve intihar riski olduğunda etkili bir tedavidir. Yeni tedavi seçeneği olarak glukokortikoid inhibisyonu çalışılmaktadır. Yineleme oranları çok yüksek olduğu için sürdürüm tedavisi önerilmektedir fakat süre ile ilgili görüş birliği yoktur.

Depression with psychotic features is one of the most serious types of depressive disorders. It is often associated with serious disability, bad prognosis and high mortality. Genetic factors reveal that depression with psychotic featured is related with both psychotic and mood disorders. It is often misdiagnosed. Treatment algorithms usually recommend antidepressants with antipsychotics or ECT as treatment options. ECT is especially recommended in patients who have suicidal thoughts and in comorbid situations. A new possible alternative treatment strategy is inhibition of glucocorticoids. Since recurrence is very often maintenance treatment is recommended but there is no consensus about duration.
